<br />Ronda Munlclpallnsurlnce Trust <br /> <br />Medical Master Plan of Beneflts <br /> <br />Participation Agreement, and who has agreed to be bound by all the terms and <br />provisions of the Trust Agreement, the Participation Agreement and the rules and <br />regulations adopted by the Trustees in the administration of the Trust. <br /> <br />HIV infection or a sDecific sickness or medical condition derived from such <br />infection means the human immunodeficiency virus identified as the causative <br />agent of acquired immune deficiency syndrome, Acquired Immune Deficiency <br />Syndrome, an acquired immune deficiency syndrome-related complex, or a <br />specific sickness or medical condition derived from such infection. <br /> <br />Home Health Agency means any state licensed public agency or private organization <br />that is equipped to provide and provides home health services. <br /> <br />Home Health Services means any or all of the following health and medical ~ervices <br />and medical supplies when furnished to an individual by a home health agency in <br />a place of residence used as a participant's home: <br />(1) part-time or intermittent nursing care provided by a Registered Graduate <br />Nurse or a Licensed Practical Nurse; <br />(2) physical, occupational, or speech therapy; <br />(3) medical social services, home health aid servIces, and nutritional <br />guidance; <br />(4) medical supplies, other than drugs or biologicals prescribed by a <br />physician, and the use of medical appliances. <br /> <br />HosDice Care means care given to a terminally ill participant by or under <br />arrangements with a hospice care agency, and must meet the standards outlined <br />by the National Hospice Association. <br /> <br />Hos~ice Care Agency means a state licensed organization and which: <br />(1) has twenty-four (24) hour hospice care available; and <br />(2) provides skilled nursing services, medical social services, psychological <br />and dietary counseling; and <br />(3) provides physician services, physical therapy, part-time home health aide <br />services and inpatient care; and <br />(4) keeps medical records; and <br />(5) has a full-time administrator. <br /> <br />Hospital means an institution which is licensed and operated in accordance with the <br />laws of the jurisdiction in which it is located pertaining to institutions identified <br />as hospitals, and which is primarily engaged in furnishing for compensation, <br />diagnostic and therapeutic facilities for surgical and medical diagnosis, which <br />provides treatment and care of injured and sick persons by or under the <br />supervision of a staff of physicians who are duly licensed to practice medicine, <br />and which continuously provides twenty-four (24) hour a day nursing service by <br />Registered Graduate Nurses, and which is not, other than incidentally, a <br />sanitarium, nursing home, place for rest, place for the aged, place for drug addicts <br />or place for alcoholics. "Hospital" also means: <br /> <br />(1) an institution which is an "ambulatory surgical center", as defined and. <br />licensed under Florida Statutes, or <br />. <br />(2) a "Psychiatric Hospital" which is an institution legally constituted and <br /> <br />..
