<br />ELIGffiILITY AND EFFECTIVE DATES <br />FORE~LOYEEDENTALCOVERAGE <br /> <br />ELIGffiILITY. An Employee becomes eligible for the coverage provided by this Policy on the later of: <br />(1) the Policy's date of issue; or <br />(2) the completion of the Eligibility Waiting Period as a full-time Employee, <br /> <br />The Eligibility Waiting Period is shown in the Schedule of BenefIts, <br /> <br />EFFECTIVE DATE, Employee Dental Coverage becomes effective on the latest of: <br />(1) the fIrst day of the Coverage Month coinciding with or next following the date the Employee becomes eligible for <br />the coverage; <br />(2) the date the Employee resumes Active Work, if not Actively at Work on the day he or she becomes eligible, The <br />Employee will be deemed Actively at Work on any regular non-working day, if he or she: <br />(a) is not totally disabled or hospital confmed on that day; and <br />(b) was Actively at Work on the regular working day before that day; <br />(3) if the Employee contributes to the cost of the Employee Dental Coverage, the fIrst day of the Coverage Month <br />coinciding with or next following the date the Employee makes written application for coverage; and signs: <br />(a) a payroll deduction order, if Covered Employees pay any part of the Policy premium for <br />Employee Dental Coverage; or <br />(b) an order to pay premiums from the Employee's Section 125 Plan account, if any contributions are <br />paid through a Section 125 Plan; or <br />(4) the fIrst day of the Coverage Month coinciding with or next following the date the Company approves a Late <br />Entrant's application, <br /> <br />GL1l-6-ELE <br /> <br />10 <br /> <br />09/01/01 <br />
