<br />City of Sunny Isles Beach <br /> <br /> <br />Data and IPT Voice Solution <br /> <br />Appendix C - Milestone/Service/Deliverables <br /> <br />In order to ensure that there is a concrete understanding of project success between <br />CTI and the Client, CTI has defined the following testing and acceptance criteria. <br />These quantifiable criteria are used to demonstrate the successful completion of the <br />required services in regard to this project. <br /> <br />Pursuant to the SOW between CTI and Customer as documented in CT1119-092104 <br />and amended by change order, Customer hereby certifies, by the signature of an <br />authorized representative, that the milestone / service / deliverable described below <br />has been completed in a satisfactory manner on the date indicated. <br /> <br />For the purpose of milestone/service/deliverable completion certification, email <br />notification of acceptance shall be acceptable. <br /> <br /> Date <br /> Oeliverables Accepted CTllnitials Client Initials <br />1 Agreed upon Detailed Design, Site Survey and <br />Pre-Plannino Document. <br /> Completed IP Telephony Acceptance Testing <br /> and formal approval of document for each <br />2 office. <br /> Provide all training and knowledge transfer as <br />3 specified <br /> Deliver all configurations and documentation <br />4 as specified under Project Manaoement. <br /> <br />If additional testing other than that listed above is required, CTI reserves the right to <br />document and incorporate a change order if appropriate. <br /> <br />The preceding tests were successfully completed by CTI and an authorized Client <br />representative and have been accepted by the Client as a demonstration of <br />completion of this project as defined in this Scope of Work. <br /> <br />Authorized Signature, CTI /Date <br /> <br />Authorized Signature, Client <br /> <br />/Date <br /> <br />Page 1 7 <br /> <br />Use or disclosure of/his information and datu is subjeclto the restriclions found on the title page of this document. <br />
