Description of proposed work: <br /> <br />a. <br />b. <br /> <br />Scope of cutting, patching, alteration, or excavation. <br />Trades who will execute the work. <br />Products proposed to be used. <br />Extent of refinishing to be redone. <br /> <br />c. <br />d. <br /> <br />6. <br /> <br />Alternatives to cutting and patching. <br /> <br />7. <br /> <br />Cost proposal, when applicable. <br /> <br />8.
