SUNNY f� <br /> °` AGREEMENT BETWEEN THE <br /> 5_ CITY OF SUNNY ISLES BEACH AND SUNBELT RENTALS INC. <br /> CONTRACT NO. C2649-1617-009 <br /> THIS AGREEMENT, entered into this 2. 4 day of OCTb6EV--- 2016, by <br /> and between the CITY OF SUNNY ISLES BEACH (hereinafter referred as to the ("City") and <br /> SUNBELT RENTALS INC., a corporation authorized to do business in the State of Florida <br /> (hereinafter referred to as the ("Contractor"), whose federal tax identification number is <br /> SIC* 04t5I9 <br /> RECITALS <br /> WHEREAS,the City is in need of a secondary contractor to provide equipment rentals for <br /> the City's various events on an as needed basis; and <br /> WHEREAS, the City issued Invitation to Bid No. 16-08-01 ("ITB"), and Contractor <br /> submitted a response thereto, a copy of which is attached hereto and incorporated herein as <br /> Attachment "A"; and <br /> WHEREAS, Contractor was selected via Resolution 2016-2601 as the second lowest <br /> responsible, responsive bidder in response to ITB No. 16-08-01; and <br /> WHEREAS, Contractor is willing and able to perform the services based on the terms and <br /> conditions set forth in ITB No. 16-08-01, a copy of which is attached hereto and incorporated <br /> herein as Attachment"B"; and <br /> WHEREAS, the City desires to enter into this Agreement with Contractor to provide the <br /> Services in an amount not to exceed Forty Five Thousand Dollars ($45,000.00) for.Athe three (3) <br /> year term. <br /> NOW THEREFORE, in consideration of the foregoing and for the mutual covenants, <br /> representations and warranties and other good and valuable consideration, the receipt and <br /> adequacy of which is hereby acknowledged, the parties agree as follows: <br /> 1. RECITALS. The Recitals set forth above are hereby incorporated into this Agreement <br /> and made a part hereof for reference. <br /> 2. SERVICES. Contractor shall provide the Services, as more particularly described in <br /> Contractor's response to ITB No. 16-08-01, which is attached hereto Attachment "A" and <br /> incorporated herein by reference. Contractor agrees to furnish all labor and materials in a good and <br /> workmanlike and professional manner. The Services shall be performed by Contractor to the full <br /> satisfaction of the City. Contractor agrees to immediately inform the City via telephone and in <br /> writing of any problems that could cause damage to the City's property, improvements and <br /> persons. <br /> 0B <br />
