<br />Manager shall require the following information: <br /> <br />(1) The type, number and complete description of all equipment <br />to be used by the applicant for providing service pursuant to this <br />chapter. <br /> <br />(2) A statement that the applicant will use Miami-Dade County <br />approved facilities for disposing of all solid waste which the applicant <br />collects and removes. <br /> <br />(3) The names of current customers and addresses of each <br />location served. Franchisees applying for renewals shall not be <br />required to submit the information specified in Section 1.03(c)(3) <br />above, but shall instead be required to submit the information specified <br />in Section 1.09. <br /> <br />(d) Insurance requirements. The applicant for a Franchise shall maintain insurance <br />as specified herein and shall furnish a public liability policy to the City and also file <br />with the City a ce~cate of insurance for all policies written in the applicant's name. <br />This certificate shall provide that the policies contain an endorsement requiring that <br />the City shall be furnished with ten (10) days' written notice by registered mail prior <br />to cancellation or material changes in the policies. <br /> <br />(1) Comprehensive general liability. The applicant shall carry in <br />his own name a policy covering his operations in an amount not less <br />than three hundred thousand dollars ($300,000.00) per occurrence for <br />bodily injury and two hundred thousand dollar's ($200,000.00) per <br />occurrence for property damage liability. <br /> <br />(2) Vehicle liability insurance. Applicants for a permit shall <br />maintain vehicle liability insurance covering each vehicle utilized in the <br />business of solid waste collection and disposal in an amount not less <br />than one million dollars ($1,000,000.00) combined single limit per <br />occurrence. <br /> <br />(3) Liability of the City. The above insurance requirements shall <br />not be construed as imposing upon the City or any official or <br />employee thereof any liability or responsibility for injury to any person <br />or property by the insured, his agents or employees. <br /> <br />(e) Proof of County and/or State Permits and Licenses. The applicant for a <br />Franchise shall provide proof of county and/or state permits and licenses <br /> <br />(f) The completed application shall be submitted to the City.
