• VI. REPORTING UNSAFE CONDITIONS AND ACTS <br />No work area can be designed and maintained as to prevent all accidents from happening. In any <br />claim or accident, at least one of two things has occurred: <br />1. There was an unsafe act (or practice), or <br />2. There was an unsafe condition <br />Employees need to know that no aspect of his or her job is so important that an unsafe condition <br />or act is permitted or authorized. <br />Each employee is to report immediately to the supervisor all unsafe acts and unsafe conditions <br />without fear of retribution. <br />Some examples of unsafe acts and conditions: <br />• Improper lifting <br />• Tampering with equipment or by- passing safety devices <br />• Smoking in hazardous /unauthorized areas <br />• Horseplay <br />• Uncovered holes and drains <br />• Frayed electrical cord /missing ground prongs <br />• Improper use of equipment <br />• Improper use of chemicals <br />• Non use /improper use of PPE <br />• Poor housekeeping <br />• Non use of warning signs <br />• Unlabelled /mislabelled chemical containers <br />SUPERVISORS MUST ENSURE EMPLOYEES ARE INSTRUCTED/TRAINED ON THE <br />ABOVE AND DOCUMENTED ON THE TRAINING CHECKLIST. <br />s <br />
