ACCIDENT INVESTIGATION • <br />The investigation and reporting of all accidents or incidents (near misses) are very important <br />elements of the CSI Safety and Loss Control Program. <br />Accident Investigation enables the Supervisor to identify the cause or causes of the <br />accident /incident, at which time positive corrective action can be taken to prevent the <br />accident /incident from recurring. <br />A GENERAL <br />1. Every accident or near miss; every illness or injury must be immediately reported <br />to the supervisor. <br />2. You and the employee should agree on the severity of the incident and determine <br />proper treatment (first aid or professional care). <br />3. Supervisor should have prior written transportation procedures to get injured <br />employee to a CSI designated treatment facility. <br />4. Supervisor or designated employee should accompany the injured to the treatment <br />facility and arrange for return transportation. <br />B. REPORTING <br />Obtain any background information available regarding both the acciednt and the <br />employee(s) concerned. 0 <br />Make a complete study of the work area and any equipment involved. Complete all <br />sections of the Supervisor's Accident Report. <br />1. Date and time of investigation. <br />2. What was the condition of the work surface? (i.e. Was the floor slipperly ?) <br />3. Was all equipment in the proper operating order? Any faulty equipment must be <br />preserved for investigation purposes. Contact Risk Management immediately. <br />4. Was material handling equipment involved? Was the operator trained and <br />authorized? <br />5. Were there any conditions in the area which may have contributed to the accident? <br />6. If equipment has been damaged or was not operating properly, has the equipment <br />been "Tagged" and removed from service? <br />T If possible, take photographs of the accident scene? <br />8. Accident must be reported to the office within 24 hours. <br />• <br />36 <br />
