Provide a narrative describing your approach to developing marketing and <br />promotional opportunities for this Project through print, radio, television and related <br />media, event sponsorships and related partnering opportunities. <br /> <br />10. Proof of authorization to transact business in Florida from the Florida Secretary of <br />State, from the prime as well as supporting firms. <br /> <br />11. The name, title, qualifications, experience, and all contact information of the <br />proposed contact person. <br /> <br />12. A listing of the proposed personnel to be assigned to this Project, including their <br />qualifications and experience. <br /> <br />13. Sworn statement pursuant to Section 287. 133(3)(A), Florida Statutes, on Public <br />Entity Crime, a copy of which is attached hereto. <br /> <br />14. Non-collusive Affidavit, a copy of which is attached hereto as Attachment "A". <br /> <br />1.7 SUBMISSION DATE AND MANDATORY PRE-RFP SUBMISSION <br />CONFERENCE <br /> <br />Submittals must be received by the City Clerk's Office no later than 10:00 a.m. on <br />Wednesday, May 6, 2009, at the Sunny Isles Beach Government Center, Attn: City Clerk, <br />First Floor Conference Room, 18070 Collins Avenue, Sunny Isles Beach, FL 33160. One <br />(1) original, and four (4) copies of the completed and executed Submission must be delivered <br />by this deadline. Submittals will be opened publicly and read aloud at this time. All contractors planning to <br />submit a Submission are required to attend this meeting. Contractors should allow <br />sufficient time to ensure arrival prior to the indicated time, those arriving after the <br />discussion has begun will not be considered to be in attendance.
