-

B. On the first pass, Crowder will begin by using crews comprised of a self-loading truck, a flagger to
maintain the flow of traffic, and a laborer to follow and monitor operations. If necessary, additional
equipment will be utilized to efficiently clear and haul away the bulk of debris on the first pass.
Trailers (with a combined capacity of 100-120 CY), rubber-tired loader, or other support equipment
could be used to aid the base crew depending on the severity of the debris.

c

C. During the first pass, Crowder will monitor the progress and capacity of the crews to determine if
additional crews are needed and what support equipment will be necessary to complete the project
within deadline.

D. Throughout the debris removal process, crews will separate debris as they go in order to reduce the
amount of non-reducible matter taken to the TDSRS. Non-vegetative or reducible C&D materials will
be sorted and disposed properly from the beginning. Roadside sorting also minimizes the risk of
hazardous waste material reaching the TDSRS in great quantities.

E. Our crews will work street-by-street, clearing debris and hauling the material to the TDSRS. Crowder
Disaster Recovery maintains a "clean as you go" policy, leaving each street it completes clean of debris
and certified as such by the company and/or CITY OF SUNNY ISLES BEACH' representatives.

3) Debris Removal from Private Property
If instructed by CITY OF SUNNY ISLES BEACH to conduct debris removal from private property,
Crowder will implement a Right-of-Entry Program. In order to collect debris on private property,
Crowder will require the owner to sign a duplication of insurance form, a Hold Harmless form, and a
Right-of-Entry form allowing the company to have access to their property. Once allowed onto the
property, Crowder will take all precautions to minimize any accidental damage that may occur.

c

Temporary Debris Staging &
Reduction Site Plan

1) Construction and Initial Set-up
Construction of the TDSR site will begin as soon as CITY OF SUNNY ISLES BEACH approves a site. Prior to
utilization, Crowder Disaster Recovery will
videotape and/or photograph the site.
Crowder will also collect environmental
samples from each site before use.

Within 48 hours of Notice to Proceed,
Crowder will have developed a specific site
plan for the TDSRS, including any road
construction needed, procedures for erosion
control, and places for portable toilets and
offices.

C Crowder will construct an observation
platform also within 48 hours according to
preliminary drawings. This tower will serve
as the primary inspection site for the TDSRS

...
