<br />Death Benefits: In the event of an employee's death, their account will be <br />transferred to the surviving spouse and/or surviving eligible dependents for <br />continuing tax-free healthcare benefits. If no spouse or eligible dependent <br />survives the employee, the account balance will remain available to the next- <br />named beneficiary for continuing healthcare benefits. Benefits paid to non- <br />spouse, non-dependent beneficiaries may be subject to income taxation, Upon <br />the death of an employee's spouse, all dependents, and all named beneficiaries, <br />any remaining assets will revert to the City. <br /> <br />HRD 9/5/06 <br />
