We had to decipher the attached. On the City's fleet listing, we noted the
items that should have inland marine and on the property schedule, the sculptures in the park
were listed as PITa in lieu of fine arts. We also scheduled the City signs.

Let me know if you have any questions.

Have a great week.

Sincerely,

Jim McGinn, CPCU, RMPE
Florida League of Cities, Inc.
Account Executive, South Florida
954.610.5153

From: Kathy Doak
Sent: Tuesday, August 30, 2011 10:12 AM
To: I Jim McGinn I
Cc: 'Priscilla Walker'
Subject: City of Sunny Isles Beach RFP # 11-08-01

Hello Jim,

Siver Insurance Consultants is reviewing the proposals provided to the City of Sunny Isles
Beach in response to their RFP # 11-0S-01 and we have several questions regarding the Florida
League of Cities/FMIT proposal.

First, so that your proposal is based on the same premium basis as the current insurer, the
City requests that you please revise your proposal so it is based on the 2009/2010 audited
payroll numbers below:

7382
7720
8810
8820
9015
9102
9402
9401

$224.400

1
