<br />Mr. Krisak has more than 33 years of experience in all phases of land and engineering
<br />surveying including topographic, boundary, hydrographic, As- Built, construction layout and
<br />location surveys for highways, residential developments commercial projects and expert
<br />witness testimony. This experience has been gained through a variety of surveying - related
<br />positions, including instrument person, drafter, party chief, and field crew coordination. He
<br />is familiar with a wide range of state -of- the -art surveying equipment. He is also experienced
<br />in project management, field survey supervision, office survey procedures, and staffpower
<br />scheduling. He serves the critical role of Quality Assurance Program Manager for the Survey
<br />Department to ensure the high quality of our survey staff and deliverables.

<br />DISTRICTWIDE SURVEYINGAND MAPPING SERVICES: Mr.Krisak served as Quality
<br />Assurance Program Manager to ensure the high quality of our survey staff and deliverables
<br />for this continuing services miscellaneous contract in which we provide the district design
<br />surveys, parcel and right of way stake outs, digital terrain models (DTM), geodetic control,
<br />control surveys, right -of -way mapping, right of way (monumentation) surveys, maintenance
<br />mapping and parcel sketches. This was a 5 -year contract, effective from October 2000 to

<br />FORT LAUDERDALE MISCELLANEOUS SURVEY CONTRACT, FORT
<br />LAUDERDALE, FL: Mr. Krisak serves as Project Manager, supervising Engineering Design
<br />Surveys, Boundary Surveys for Key Projects such as Sistrunk Boulevard, NW 19th Street,
<br />Miami Road, Sunset Memorial Gardens, Septic Area #2, #3, #4, #5, #8 and #16, Facility
<br />Security Sites, Fort Lauderdale Country Club, Shady Banks, Starlight Landing, Fire Station # 13,
<br />Sunrise Key and Sunrise Intracoastal Subdivision, NE 56th Street, South Andrews Avenue of

<br />JEA VARIOUS PROJECTS: Mr. Krisak was Project Manager for various Citywide Projects
<br />such as University Boulevard Water Main Replacement, Day Avenue Force Main, Phelps
<br />Street to Florida Avenue Survey, Racetrack Road Right -of -Way Survey, North JAX Force
<br />Main, Lakeshore Boulevard Survey and Bush Road Survey.The major survey tasks provided
<br />on these projects were Topographic Survey, Coordination of Subsurface Utility Designation,
<br />Design Survey, Coordination of Utility Locations, Sketch and Description of Easements.
<br />TRI- RAIL /CSX DOUBLE TRACKING PROJECT, PALM BEACH, BROWARD,
<br />AND MIAMI -DADS COUNTIES, FL: Mr. Krisak served as Project Manager for this
<br />project which involved supplying over 40 miles of control, design and as -built surveys along
<br />the CSX corridor.The surveys were completed to current FDOT standards. (2006)
<br />CSX RAILROAD, MIAMI -DADS, BROWARD, AND PALM BEACH COUNTIES,
<br />FL: Geodetic survey and right -of -way mapping for the CSX Railroad from Miami -Dade
<br />County to Palm Beach County. (1988)
<br />MIAMI MOVER, MIAMI -DADS COUNTY, FL: Mr. Krisak serves as Project Manager
<br />supervising verification of horizontal and vertical control for this project. Also, as- builts of
<br />newly constructed columns, pier caps, joints and plinths from intermodal center to terminal
