The bascule piers are supported on 1,22m- diameter drilled shafts. The superstructure type for the approach spans is a <br />precast concrete segmental box girder and a substructure consisting of cast -in -place or precast concrete piers supported on a <br />F� cast -in -place foundation cap with 1.22m- diameter drilled shafts. The span length for the bascule span is 56m between centerline of <br />trunnions and 61 m between the centerline of foundations of the bascule piers. This span arrangement is necessary to provide the <br />required horizontal clearance of the channel. The span arrangement for the west side approach is a three span unit consisting of <br />j spans of 39.0m, 36.570m, and 30.543m with the longest span adjacent to the bascule span. The east side approaches consist of a <br />J five span unit with spans of 39.0m, 36.570m, 31.045m, 25.175m, and 18.738m. The total length of each bridge is 332.641 m. <br />r Mr. Jacob was responsible for ensuring project was constructed as per plans and specifications. This project is to <br />construct for the widening of 14 miles, beginning at the Homestead Extension of the Turnpike, to provide three lanes in each <br />r, direction. Work included roadway and bridge widening, drainage improvements, overhead signing, high resistant lighting, <br />landscaping and the construction of two new toll plazas. Responsibilities included: depth check, string line, densities of stabilized <br />f subgrade and optional base groups, asphalt documentation and concrete placement and testing. This new <br />$24- million bascule bridge project consisted of total reconstruction - demolition of the existing bridge and reconstruction of a new <br />bascule bridge which had many unique features including architectural precast features, inverted concrete T -beams for the <br />approach spans and Tensar retained earth wall approaches. This was the first use of the unique shape of these concrete T- <br />beams in Florida. Responsibilities included overseeing the preparation of all change orders, supplemental agreements, work <br />orders, final estimates documentation, Construction Quality Reporting (CQR) data input, monthly pay estimates and all other <br />contract paperwork requirements. FDOT Project Manager: Deborah Ihsan; (561) 432 -4966. <br />2198 — 9198, Senior Inspector, Oakland Park Avenue Reconstruction of In and Out Ramps and Toll Plazas to Sawgrass <br />Expressway, Florida's Turnpike Enterprise, Ft. Lauderdale, Florida. This project involved the reconstruction of in and out <br />ramps to Sawgrass Expressway and new construction of the toll plaza on each ramp. Responsibilities included inspections of <br />work performed, recording and taking samples, involving earthwork, street lighting, signalization, concrete placement and asphalt <br />paving operations. FDOT Turnpike Project Manager: Don Cronk; (954) 975 -4855. <br />5197 — 2198, Treshold Inspector, L'Hermitage East Tower, Gopman Consulting Engineers, Inc., North Miami Beach, Florida. <br />This project is to build a 28 story building above habitable plane (17' -0 ") with 220 unit condominiums, balconies, 3 stories parking <br />facilities and landscaped swimming pool. The total building area is 856,672 SQ. FT. Mr. Jacob was responsible for ensuring <br />project was constructed as per plans and specifications. Responsibilities also included but not limited to construction inspections of <br />rebars, every post- tensioned cable, concrete materials and for any construction related to building shell construction. Structural <br />Engineer /Designer: Mr. Herman Gopman; (305) 932 -5911. <br />10195 — 11/96, Inspector, LeJeune Road Interchange, FDOT District VI, Miami, Florida. This project involved the construction <br />of new ramps between SR -836 and N.W. 45th Avenue and the reconstruction of SR -836, LeJeune Road and the vicinity of the <br />Miami International Airport. Responsibilities included density taking and maintaining the density log book for embankment, storm <br />drainage installation, asphalt placement over new limerock base, curb and gutter, signs and sign structures, and construction of <br />MSE walls. Measured and verified quantities of materials for final estimate, FDOT Project Manager: Stu Smith; (954) 975 -4855. <br />2192 — 9195, Engineer I, Florida Department of Transportation, Florida. Responsible for field inspection of projects under <br />construction to include progress record testing and sampling of soils in place and comparing results and procedures with <br />established criteria. Checked project records pertaining to density for accuracy and completeness. Ensured compliance with <br />TARGET ENGINEERING GROUP, INC. <br />
