BID OFY ZOQ¢MM0 CITY OF HALLANDALE BEACH PAGE 31 OF35 <br />FLORIDA <br />SPECIFICATIONS <br />THIS 0 NOT AN ORDER <br />ATTAMMENT <br />INSURANCE REQUIREMENTS - SUPPLIERS <br />Suppliers shall procure and maintain for the duration of and in full compliance with the contract <br />insurance against claims for injuries to penions and damage to property which may arise from or in <br />connection with the delivery and supply of products, materials, and/or services to the City by the <br />supplier, his agents, representatives and employees. The cost of such insurance shall be borne by the <br />supplier. <br />A. Commercial General Liability, including: <br />1. Premises and Operations. <br />2. Products and Completed Operations Coverage. <br />3. Blanket Contractual Liabiltty Coverage, <br />(see Hold Harmless Agreement and AM I below). <br />4. Independent Contractors. <br />5. Broad Form Property Damage. <br />S. Personal Injury Liability. <br />7. Fine Legal Liability Coverage. <br />8. Incidental Medical Malpractice Coverage. <br />B. Automobile Liability Insurance, including: <br />1. Owned Automobiles. <br />2. Nonowned Automobiles. <br />3. Hired Automobiles. <br />C. Workers' Compensation Insurance. <br />D. Employers Liability Insurance. <br />
