6. Employees who are on an approved leave of absence may not perform work for any <br /> other employer during the leave, except when the leave is for military or public service <br /> or as allowed by federal or state law. Employees found to be performing work for any <br /> other employer shall be separated from employment with the City. <br /> 7. Sick leave request and requirements: <br /> a. Records of sick leave used will be kept by the Human Resources Director, or <br /> his/her designee(s), Department Heads, and Payroll on prescribed forms and will <br /> be monitored for adherence to policies. pursuant to the procedures outlined in the <br /> ' 1 _ ,I. • - <br /> ee <br /> b. It is the employee's responsibility to notify his or her supervisor as far in advance <br /> as possible of any absence. Failure to give such timely notification, without good <br /> cause, may forfeit any claim for sick pay. <br /> c. When an employee is absent due to illness for a period longer than three (3) days, <br /> he or she may be required to submit medical evidence of the reason for the <br /> absence (medical certification), upon request by his or her Department Head. If an <br /> employee has a record of excessive one-day absences (especially those in <br /> conjunction with scheduled time off for weekends) or on Mondays or Fridays, the <br /> employee may be required, at the discretion of the Department Head, to submit <br /> medical certifications for even one-day absences. <br /> d. Medical, dental, and vision appointments will be charged to sick leave for the actual <br /> time used in amounts not less than one (1) hour. <br /> e. Employees who are ill and who will miss work that day are required to speak <br /> directly with their immediate supervisor (not a support person or answering <br /> machine) prior to the start of their work shift. If the supervisor is unavailable, the <br /> employee should attempt to contact the next person in their chain of supervision. <br /> If neither is available, the employee should call back later in the work shift to <br /> contact the supervisor and/or contact the department head, or Human Resources <br /> Director (during normal work hours). Employees are required to call in every day <br /> thereafter if they are still sick and will continue to miss work, except in instances of <br /> prolonged illness (more than two (2) weeks) or when a medical note has been <br /> provided indicating the amount of time the employee will be out.
