City of Sunny Isles Beach <br /> Personnel Policy and Procedure <br /> Policy Name FAMILY AND MEDICAL LEAVE (FMLA) (NEW) <br /> Policy Number 704:2 <br /> Effective Date 5/18/17 <br /> Revision Date(s) <br /> Prepared By, Title Human Resources Director <br /> Approved By, Title City Manager <br /> PURPOSE: <br /> To establish a procedure for the review and approval of the use of leave pursuant to the <br /> provisions of the Family and Medical Leave Act of 1993 (FMLA) and the National Defense <br /> Authorization Act of 2010. This policy is consistent with the federal act and shall be <br /> interpreted accordingly. <br /> POLICY: <br /> It is the City's intent to provide leave to eligible employees in compliance with the Family <br /> and Medical Leave Act of 1993 (FMLA) and the National Defense Authorization Act of <br /> 2010. The following provisions summarize the requirements of the FMLA and the National <br /> Defense Authorization Act of 2010 as they apply to City employees. Changes in the <br /> Family and Medical Leave Act of 1993 (FMLA) or the National Defense Authorization Act <br /> of 2010 will result in changes to these provisions. In the event of any inadvertent conflict <br /> between this policy and the requirements of Family and Medical Leave Act of 1993 <br /> (FMLA) or the National Defense Authorization Act of 2010, the requirements of Family <br /> and Medical Leave Act of 1993 (FMLA) or the National Defense Authorization Act of 2010 <br /> will apply. <br /> ELIGIBILITY: <br /> 1. Full time employees who have been employed for at least 12 months and have <br /> worked at least 1,250 hours during the previous 12-month period before the date <br /> the absence begins. <br /> 2.
