will have seven calendar days to correct any deficiency in the certification. <br /> If it is not practicable under the particular circumstances for the employee <br /> to cure any deficiency in the seven-day period despite the employee's <br /> diligent good faith efforts, the City may provide additional time. <br /> 3. If an employee fails to provide a complete and sufficient certification despite <br /> the opportunity to cure the deficiency, the City may deny the employee's <br /> request for FMLA leave. <br /> 4. If the employee or employee's family member is visiting another country, or <br /> a family member resides in another country, the City will accept a medical <br /> certification, including second and third opinions, from a health care <br /> provider who is authorized to practice in that country and is performing <br /> within the scope of his or her practice. If a certification by a foreign health <br /> care provider is not in English, the employee must provide a written <br /> translation at the employee's expense. <br /> Certification for Covered Servicemember (Military Caregiver Leave) <br /> 1. Leave to care for a covered servicemember with a serious injury or illness <br /> must be supported by a certification completed by a Department of Defense <br /> (DOD), Veterans Affairs (VA), or TRICARE health care provider, or by a <br /> private health care provider or an Invitational Travel Order (ITO) or <br /> Invitational Travel Authorization (ITA). <br /> 2. The Human Resources Department is permitted to obtain the name and <br /> contact information of the health care provider, the type of medical practice, <br /> and whether the provider is "authorized." <br /> 3. Information may also be obtained on whether the injury occurred in the line <br /> of duty, when it occurred, its probable duration, and other information <br /> sufficient to determine need for care, and the amount of time the <br /> servicemember will require care. <br /> 4. The Human Resources Department may also request information from the <br /> employee on the relationship between the employee and the <br /> servicemember, the covered servicemember's military branch, and <br /> information concerning the covered servicemember's condition and care. <br /> Certification of Qualifying Exigency for Military Family Leave <br /> 1.
