<br />City of Sunny Isles Beach |Request for Proposal No. 17-10-02 10 <br /> <br />2.5 PRICE <br /> <br />If the Proposer is awarded a contract under this solicitation, the prices proposed shall remain fixed <br />and firm during the initial term of contract. However, the Proposer may offer incentives and <br />discounts from this fixed price to the City at any time during the contractual term. No additional <br />costs may be accepted, other than the costs stated on the attachment A, Compensation Proposal. <br /> <br />2.6 TAXES/LICENSES <br /> <br />Retail taxes and rental taxes and licensing are the responsibility of the Contractor and shall be <br />handled in accordance with Florida State Statutes. The successful contractor shall be responsible <br />for, at its own expense, obtaining all necessary permits and pay ing all licenses, fees and taxes, <br />required to comply with all local laws and ordinances; county, stat e and federal laws, rules and <br />regulations which are applicable to the business to be carried on under this contract. Successful <br />proposer shall be responsible for all sales tax, licenses, fees, etc., and shall not deduct such from <br />the percentage revenue fee owed to the City. <br /> <br />2.7 DELIVERY <br /> <br /> Unless otherwise indicated, all items shall be delivered F.O.B. with destination and delivery charges <br /> included in the proposal price. F.O.B. destination is interpreted to mean unloading and placing in <br /> the building or area as directed by the City. <br /> <br />2.8 INSURANCE <br /> <br /> Comprehensive General Liability with minimum limits of five million dollars ($5,000,000.00) per <br />occurrence for Bodily Injury and Property Damage Liability. Coverage must be afforded on a form <br />no more restrictive than the latest edition of the Comprehensive General Liability policy, without <br />restrictive endorsements, as filed by the Insurance Services Office, and must include: <br /> <br />o Premises and Operation <br />o Independent Contractors <br />o Products and/or Completed Operations Hazard <br />o Broad Form Property Damage <br />o Broad Form Contractual Coverage applicable to this specific contract, including any hold <br />harmless and/or indemnification agreement. <br />o Personal Injury Coverage with employee and Contractual Exclusions removed, with <br />minimum limits of coverage equal to those required for Bodily Injury Liability and Property <br />Damage Liability. <br /> <br /> Business Automobile Liability with minimum limits of one million dollars (1,000,000.00) per <br />occurrence combined single limit for Bodily Injury Liability and Property Damage Liability. <br />Coverage must be afforded on a form no more restrictive than the latest edition of the <br />Business Automobile Liability policy, without restrictive endorsements, as filed by the <br />Insurance Services Office, and must include: <br />
