<br />City of Sunny Isles Beach |Request for Proposal No. 17-10-02 24 <br /> <br />information shall be provided on the Contractor’s Project References page <br />attached to the Bid Form, and shall be submitted at the time of the bid. <br /> <br />3. STAFFING <br />Relevant experience and qualifications of key personnel, including key personnel of <br />subcontractors, that will be assigned to this project as well as experience and qualifications. <br />The size and experience of the company staff pool from which staff assigned to the contract <br />can be drawn. The composition of the staff team should include: <br /> <br /> The names of the employees in the area responsible for this contract <br /> Their function in the company. <br /> The name of the person who will be responsible for the coordination of work. <br /> <br />4. APPROACH / METHODOLOGY <br />Proposer’s approach methodology to providing the services requested in this solicitation <br /> <br /> Suitability of the methodologies and approaches used in achieving tasks <br /> Overall organization to completing the project <br /> Ability to meet desired timelines and deadlines <br /> The ice rink contractor shall submit complete shop drawings for all equipment <br />and materials to the Owner prior to fabrication and installation. <br /> All submittals shall be in conformance with General Requirements and Submittal <br />Procedures. <br /> Complete fabrication shop drawings of the entire system, accessories and <br />equipment. <br /> Literature and Drawings, including dimensions, weights, required electrical wiring <br />and control diagrams. <br /> Recommendations, procedures and requirements for the coordination with the <br />work of other trades on the Projects, completed wiring, piping and control <br />diagrams covering field installation. <br /> <br />5. COST PROPOSAL – Include on Attachment A <br /> <br /> Payment Terms – such as, initial deposit and payment schedule per project phases <br /> 1. Total fee for completion of the installation including labor and equipment. <br /> 2. A list of anticipated reimbursable expenses and the rate charged for each. <br /> 3. Monthly maintenance fee <br /> 4. Professional Emergency Service Fee (Hourly Rate) <br /> 5. Any reduced fees offered to other municipalities, governmental entities <br /> 6. Miscellaneous cost/fees must be included. <br /> 7. In addition, the vendor should also include a cost breakdown of the estimated profit <br /> returned to the City.
