<br /> Alpine Towing, Inc., d/b/a Galactic Towing, hereinafter referred to as "Alpine Towing", is a Florida
<br /> Corporation, organized on February 10, 2006. Please refer to the attached print out from Sunbiz.org
<br /> regarding our Corporate Status with the State of Florida and a copy of our current Towing License
<br /> (TL#2656) issued by RER. Alpine Towing, Inc., is fully insured with Vehicle Liability, On-hook Cargo, and
<br /> Garage Keepers Insurance and Workers Compensation Insurance. Please refer to the attached
<br /> Certificates of Insurance for complete explanation and coverages of insurance.
<br /> Alpine Towing is regularly engaged in providing towing services and storage of impounded vehicles,
<br /> requested by the Miami-Dade County Police Department, City of Doral Police Department, El Portal
<br /> Police Department, Federal Bureau of Investigation, Florida Fish & Wildlife Conservation Commission,
<br /> Miami-Dade County Internal Services Department, Sweetwater Police Department (as back-up) and
<br /> many private property owners including but limited to: Apartment Buildings; Condominiums; and
<br /> Shopping Centers; and various other small business type properties.
<br /> All towing service is provided in accordance with regulations contained in Chapter 30, Article III, titled
<br /> "Towing of Motor Vehicles" of the Code of Miami-Dade County. In addition to these County
<br /> Ordinances, Alpine Towing, Inc., follows Florida Statutes 713.78 and 715.07 and all supporting sections
<br /> and subsections.
<br /> Alpine Towing, Inc., also provides Towing Services for many Roadside Assistance Companies, including
<br /> GEICO, Road America, Agero, Allstate, and Cross Country. Each roadside assistance company has
<br /> requirements including but not limited to: charges for services rendered, mileage, and storage (if
<br /> applicable). Invoices are monitored and verified by each respective Company for accuracy of charges,
<br /> vehicle information, and vehicle owner information.
<br /> Alpine Towing, Inc., has a staff of 27 employees consisting of Owners, dispatchers, drivers and support
<br /> staff. Each employee is leased from FrankCrum Corporate, Inc., based at 100 South Missouri Avenue,
<br /> Clearwater, FL 33756. In addition, please refer to the schedule of employees currently covered by
<br /> Workman's Compensation Insurance.
<br /> Alpine Towing has been licensed with the Miami-Dade County under TL#2656, since February 10, 2006.
<br /> Alpine Towing has current Business Tax Receipts for all municipalities which are required by local City
<br /> ordinance, to provide towing services. Through the date of submittal, Alpine Towing has had citations
<br /> issued by RER for drivers towing vehicles without safety chains, but has not had any issues with
<br /> paperwork or charges to the customer. Each year the RER does an annual inspection, for the TL
<br /> ® renewal and each office has not had any issues with required notices to customer, types of payment
