RFP# 12-10-03 ø4k4aLJQp� ` Quote# 1212-212 Artificial Turf for Senator Gwen Margolis and Pelican Community Parks City of Sunny Isles Beach Date: December 12, 2012 RE: Request for Proposal No. 12-10-03 Quote: 1212-212 The following is a proposal by Medallion to supply and install artificial turf at Senator Gwen Margolis and Pelican Community Parks on North Bay Road in Sunny Isles Beach, FL. Proposed work to include replacing the entire playing surface within the fenced enclosure at Senator Gwen Margolis Park with artificial turf, permanently lined for soccer play. Proposed work also includes replacing the playing surface within the fenced baseball field enclosure at Pelican Community Park with artificial turf. Upon acceptance of this proposal, Medallion will provide all materials, equipment and/or labor as outlined below. The scope of this proposal shall include the following: SCOPE OF WORK • • Professional services include initial survey, field design and layout, construction and erosion control permitting. • Supply and install required construction entrances and erosion control measures. • Medallion to excavate soil to 0.5% crown. Spoils to be relocated off-site. • Medallion to fine grade sub-base at proposed artificial turf areas only. Sub-base to be quality assured with proof roll. ® • Medallion to install collector pipe at perimeter of turf areas (Assumes that Medallion can tie into existing storm drain within 50ft of each field). • Supply and install non-woven fabric over entire sub base and under perimeter trench drains at fields. Back-fill trenches with stone. • Install concrete curb and nailer board at perimeter of each field. • Supply and install Safety Curbing along perimeter of pitching mound and batter's box at baseball field. These areas are to remain day soil. • Supply and install layers of 2"and 4" laser graded stone base at each field. • Supply and install 20mm Sport Drain Max shock and drainage pad over stone base • Install approximately 55,750sgft of Shaw Sports Turf Powerblade Elite 45 (1.75" pile) with combination SBR (crumb rubber) and sand infill at Senator Gwen Margolis Park. Turf color to be chosen from standard color line. • Supply and install inlaid gamelines for soccer at Senator Gwen Margolis Park. Color to be determined from standard color line. • Install approximately 40,650sgft of Shaw Sports Turf Powerblade Elite 45 (1.75" pile) with combination SBR (crumb rubber) and sand infill at Pelican Community Park. Pitching mound and batter's box shall remain clay soil. Interior infield, outfield, and bordered areas shall be green turf. Infield baselines, infield foul area, and warning track shall be clay tone turf. Colors shall be chosen form standard color line. Clay tone turf color for infield, warning track, and baselines shall be chosen from standard color line. • Supply and install inlaid gamelines for baseball at Pelican Community Park. Color to be determined from standard color line.
