Actions taken to mitigate future occurrences of the deficiencies <br /> <br /> Contractor must provide Project Manager(s) with a hard copy and/or electronic copie s of <br /> all forms and documents prepared as part of the Quality Control Plan monitoring. <br /> <br />2.20.2 FIELD SUPERVISOR <br /> The Contractor must provide 1 qualified on-site full-time working Field Supervisor to <br />manage Contractor’s personnel at the Work sites. One (1) Field Supervisor shall be <br />assigned to the Cultural & Community Services Department. These people (and their <br />substitutes) must have full authority to act for the Contractor on all matters relating to the <br />daily performance of the Work at the Work site(s). The Field Supervisor must be the <br />central point of contact in the field for the City; and, must effectively communicate in <br />English. The Field Supervisor must understand and be able to fulfill, completely and clearly, <br />the Performance Standards and Technical Specifications and reporting requirements of the <br />Contract. A resume of the Field Supervisor must be submitted with the Contractor’s <br />Qualifications as specified in Section 7, which must include all contact information (i.e. <br />telephone, email address, cell phone, etc.). The field supervisor must have a minimum of <br />five (5) years combined horticultural education and experience as a landscape field <br />maintenance supervisor on contracts of similar size, scope, and complexity, and must <br />remain on-site at all times while Work is being performed under the Contract. When the <br />on-site working Field Supervisor is absent for the day or for an extended period (more than <br />4 hours), the Project Manager(s) must be notified and the Contractor must appoint a <br />qualified substitute. Within five (5) days of execution of the Contract, the Contractor must <br />provide the Project Manager(s) with a cellular phone number for the Field Supervisor where <br />he/she can be reached at all times. <br /> <br />2.20.3 The Contractor must provide a minimum of six (6) full time staff, one (1) on-site Field <br />Supervisor Monday – Friday and two (2) full-time staff during weekends with (1) one on- <br />site Field Supervisor (or additional staff as needed to meet scope of service, as determined <br />by the City). All Work shall be accomplished anytime between the hours of 7:00 A.M. to <br />5:00 P.M. Monday through Sunday. The minimum required staff is for routine landscape <br />maintenance services only and does not include services such as annual flower change- <br />outs, shrub replacement, special projects, irrigation repair, tree and palm pruning . The <br />Contractor is responsible to provide additional staff as needed to perform this work and all <br />other work contained in this ITB. <br /> <br />The City shall not be liable for any overtime costs for any staff working in excess of eight <br />(8) hours per day or (40) hours per week in order to accomplish the Work in accordance <br />with the Performance Standards. <br />
