<br />City of Sunny Isles Beach | ITB 18-03-03 Right-of-Ways and Facilities Landscape Maintenance Services 26 <br /> <br />(1) hour notification for emergencies at all times at no charg e, including facility <br />maintenance duties, boarding up of facilities, and other work as directed by the City, shall <br />be present at up to 3 special events per year as requested by the City, and be present at <br />all planning meetings at no charge, with twenty-four (24) hour notice. This person shall <br />perform inspections at minimum twice per week of the landscaped areas in the scope of <br />services and be available to City for routine and as-needed inspections as directed by the <br />City, shall be available to the City’s Project Manager(s) and Consultant as directed by the <br />City for inspections, reviews, and consultation via telephone or other approved wireless <br />communications on a 24/7/365 basis. When this person is out of City on vacation or leave, <br />an alternate contact shall be provided, and shall be approved at the City’s sole discretion. <br />The Program Manager must be available to discuss Contract matters and performance <br />issues with the Project Manager(s) or Consultant during regular City business hours and <br />within one (1) hour during other times. This can be accomplished in person, or by <br />telephone, as appropriate depending on the circumstances. The duties will be performed <br />at no additional labor or travel charges. The Contractor must provide to the Project <br />Manager(s) the contact information (i.e. phone, email address, cell phone, etc.) for the <br />Program Manager and an alternate individual, within five (5) days of execution of the <br />Contract. A resume of the Program Manager must be submitted with the Contractor’s <br />Qualifications as specified in Section 7, which must include all contact information (i.e. <br />telephone, email address, cell phone, etc.). The Contractor must provide an updated list <br />no less than five (5) days of any changes of the Program Manager or contact information <br />provided. Foreman, Field Supervisors, Account Managers, etc. are not acceptable. <br /> <br /> The Program Manager must furnish a monthly report (“Report”) to the Project Manager no <br /> later than the fifteenth (15 th) of each month that must consist of five (5) parts, broken <br /> down as follows: <br /> <br /> Part 1: Prior month’s Basic Services activities accomplished, identified by park, right -of- <br />way, beach, beach access path, bus shelter or government facility and must include the <br />date(s) the Work was performed. <br /> <br /> Part 2: Prior month’s Supplemental Services activities accomplished, identified by park, <br />right-of-way, beach access path, bus shelter, parking lot, or government facility, the <br />date(s) the Work was performed and the cost(s) associated with the Work. <br /> <br /> Part 3: Prior month’s Re-Work activities accomplished, identified by park, right-of-way, <br />beach access path, bus shelter, parking lot, or government facility, the date(s) the Work <br />was performed. <br /> <br /> Part 4: Prior month’s Additional Services activities accomplished, identified by park, right- <br />of-way, beach access path, bus shelter, parking lot, or government facility, the date(s) the <br />Work was performed. <br /> <br /> Part 5: Prior month’s inspections conducted under the QCP. This Part of the Report must <br /> include the following: <br />
