<br />City of Sunny Isles Beach |RFP Disaster Debris Management and Disposal Services 18-04-02 34 <br /> <br />prior to beginning work. At a minimum, the plan will address the following: <br /> <br />a) Access to the site. <br />b) Site management, to include point of contact, organizational chart, etc. <br />c) Traffic control procedures. <br />d) Site security. <br />e) Site safety. <br />f) Site layout/segregation plan. <br />g) Environmental mitigation plan, including considerations for smoke, dust, noise, traffic, <br />buffer zones, and storm water run-off as appropriate. <br /> <br />DMC shall reclaim each TDMS to its pre-use condition within thirty (30) calendar days of receiving <br />the last load of disaster-related debris. Closure shall include, but not be limited to, removal of all <br />equipment and debris, grading the site to historical conditions, seeding and mulching of exposed <br />areas, repairing irrigation fences and roads, and removing all remnants from the processing <br />operation (such as temporary toilets, observation towers, security fence, etc.). The site will be <br />restored in accordance with all local and contractual requirements. <br />3.19 HAUL-OUT OF REDUCED DEBRIS TO CITY APPROVED FINAL DISPOSAL SITE <br /> <br />Services performed under this Contract element will be compensated using Attachment A – Unit <br />Rate Price Schedule 2. The DMC shall provide the name and address of each disposal facility to be <br />used along with the name and the telephone number of a responsible party for each facility, prior <br />to commencing the work. The Contractor shall not use any disposal facility without the written <br />consent of the Contract Manager. At the completion of disposal operations, each disposal facility <br />will issue a written summary of the quantity, type and origin of waste delivered. The Contractor <br />shall not receive any payment from the City for haul-out or load tickets related to reduced or <br />unreduced debris transported and disposed of at a non-City approved Final Disposal Site. <br /> <br /> DMC shall provide documentation for all items salvaged or recycled. Document ation shall include <br /> identification of material type, quantity, location material is accepted for salvage or recycling, and <br /> the value of the salvaged or recycled material. DMC shall provide the value of the salvaged or <br /> recycled material back to the City as a reimbursement credit back to FEMA, as required by <br /> FEMA. The value of the material will be defined as the value of the material as paid to DMC by <br /> the entity accepting the material for salvage or recycling. <br /> <br />3.20 ADDITIONAL AS NEEDED SERVICES <br /> <br />DMC may be requested to perform the services detailed below , compensated under Schedule 4: <br /> <br />a. Marine Debris Removal – DMC shall clear waterways of debris and fallen trees as <br />identified and directed by the City in writing. DMC shall ensure all work is eligible and <br />documented in compliance with FEMA or NRCS requirements for reimbursement.
