<br />City of Sunny Isles Beach |RFP Disaster Debris Management and Disposal Services 18-04-02 43 <br /> <br />will be employed by the Contractor to support the needs and objective of the City. <br /> <br />B. Provide a statement demonstrating an understanding of the services and support <br />required by this RFP. State how the Contractor will approach the project and the <br />methodology to be used to perform the services described in the Scope of Services. The <br />technical approach should also outline the following: <br /> <br />1. Ability to manage activation of multiple contracts <br />2. Methods for mobilization/demobilization <br />3. Operational plans and work procedures <br />4. Documenting and resolving damages <br />5. Invoicing and data management <br />6. Identify any issues or concerns of significance that may be appropriate. <br /> <br />C. Completed Disclosure of Sub-Contractors: Include a Sub-Contracting plan that identifies <br />items such as a description of percentage of work to be subcontracted. <br /> <br />D. Describe Contractor’s ability to avoid and/or mitigate unforeseen problems such as <br />equipment failure and staffing shortages. This includes, but is not limited to ongoing <br />maintenance programs, availability of parts and personnel for field repairs, resources for <br />backup personnel and equipment, and other programs and approaches that would allow <br />the Contractor to meet the City’s needs and objectives in adverse conditions. <br /> <br />E. Exceptions – clearly describe any exceptions the Contractor may have in regards to any <br />requirements stated in the RFP document or associated addendums. <br /> <br />F. Identify Contractor’s current inventory of heavy equipment, vehicles and other related <br /> equipment and their current conditions that would be dedicated and utilized for the City’s <br /> objective. A full list shall include descriptions, sizes and age of the equipment. <br /> <br />9. Price Proposal: <br /> <br /> A. Proposal form shall be signed by the contact person authorized to represent the <br /> contractor. The contractor must provide their pricing through the designated line items <br /> listed on the Price Form. Proposals shall contain an acknowledgment of receipt of <br /> all Addenda. <br /> <br /> <br /> <br /> <br /> <br />END OF SECTION
