<br />City of Sunny Isles Beach | Invitation to Bid No. North Bay Road Drainage Improvements 18-04-01 25 <br /> <br />The City retains the right to request any additional information pertaining to the <br />Contractor’s ability, qualifications, and procedures used to accomplish all work under the <br />contract as it deems necessary to ensure safe and satisfactory work. <br /> <br /> 6. Corporate Standing and Authorized Signatory <br /> <br />Respondent must demonstrate that the company is in good standing and that the person <br />signing this submittal is an Authorized Signatory on behalf of the respondent to sign bids, <br />proposals, negotiate and/or sign contracts, agreements, amendments and related <br />documents to which the respondent will be duly bound. The respondent must provide a <br />copy of the State Certificate of good standing listing the officers of the company. If the <br />signatory is not one of the officers listed on the State Certificate, the respondent must <br />provide one of the following forms of evidence of Signatory Authority with its response: <br /> <br />a. A copy of firm’s Articles of Incorporation listing the approved signatories of the <br />corporation; <br /> <br />b. A copy of a resolution listing the members of staff as authorized signatories for the <br />firm; and <br /> <br />c. A letter from a corporate officer listing the members of staff that are authorized <br />signatories for the firm. <br /> <br /> 7. Litigation History <br /> <br />CONTRACTOR shall provide a summary of any litigation or arbitration that the <br />CONTRACTOR, its parent company or its subsidiaries have been engaged in or are currently <br />engaged in, during the past five (5) years against or involving (1) any public entity in <br />Florida for any amount (2) any private entity for an amount greater than $100,000. The <br />summary shall state the nature of the litigation or arbitration, a brief description of the <br />case, the outcome or projected outcome, and the monetary amounts involved. The CITY <br />may disqualify any CONTRACTOR it determines to be excessively litigious. <br /> <br />8. Forms and Attachments <br /> <br />Enclosed Forms; <br />Respondent must complete, sign as required, and submit the Addenda and all forms <br />Firm’s Current Certificate(s) of Insurance; and Attach a financial statement including <br /> bidder’s latest balance sheet and income statement. <br /> <br /> <br /> <br />END OF SECTION <br /> <br /> <br /> <br />
