Additional required submittals, by Section, include, but are not limited to: <br /> <br /> 01000: Summary of Work <br /> Notice of Intent to Use the Generic Permit for Discharge of Ground Water from <br />Dewatering Operations <br /> 03500: Well Development <br /> Daily log development sheets <br /> <br />1.5 SCHEDULE OF VALUES <br />A. The Contractor shall submit a Schedule of Values for OWNER’s Approval in accordance <br />with Section 01025 entitled "Measurement and Payment" which shall include all items in <br />the bid proposal including those items which are identified as lump sum. The lump sum <br />items shall be broken down into component parts for the purpose of tracking monthly <br />progress of the work and making monthly progress payments. The schedule shall contain <br />the installed value of the component parts of Work for the purpose of making progress <br />payments during the construction period. <br /> <br />B. The schedule shall be given in sufficient detail for the proper identification of the Work <br />accomplished. Each item shall include its proportional share of all costs including the <br />Contractor's overhead, contingencies, and profit. The sum of all scheduled items shall <br />equal the total value of the Contract. The sum of the component parts shall equal the <br />lump sum price of the lump sum item in the bid form. <br /> <br />C. The Contractor shall expand or modify the above schedule and materials listing as <br />required by the OWNER's initial or subsequent reviews. <br />1.6 24 HOUR EMERGENCY CONTACT LIST <br />A. The Contractor shall be required to provide emergency contact telephone numbers for <br />this project. A minimum of two 24-hour contact telephone numbers are required including <br />a 24 hour contact for MOT maintenance if there is an overnight maintenance of Traffic <br />plan in place. Telephone numbers are required for the following Contract positions: <br />1. Project Manager <br />2. Project Superintendent <br />3. Maintenance of Traffic Supervisor (24 Hr) <br /> <br />Note: The OWNER must have two 24-Hour contact telephone numbers, one which includes either <br />the Project Manager or Project Superintendent.
