modeling and monitoring for advance forecast and readiness plan/planning as assigned may be <br /> requested by the City. <br /> 8.10.1 Loading Site Monitoring <br /> 1. DP&O will supply load site monitors; the primary function of the Loading Site Monitors is to <br /> approve debris load tickets for eligible debris cleared and removed at locations designated by the <br /> Debris Management Center. <br /> 2. DP&O will, with 24 hours of notification, be prepared to provide qualified on-site personnel to <br /> monitor debris removal operations throughout the City. Additional sites may be added as debris <br /> removal efforts increase. Each loading site will operate, at a minimum, approximately 12-14 <br /> hours per day, 7 days per week. Exact number and location of loading sites will be determined <br /> by the Debris Manager in coordination with the debris removal Contractor. <br /> a. DP&O may have Loading Site Monitors stationed at designated "Control Points" chosen by <br /> the debris removal Contractor and coordinated with the Debris Management Center the day <br /> before beginning the work. The number of"Control Points" must be kept to a minimum and be <br /> located at a safe site along the primary haul road to the designated debris management site. <br /> DP&O must be prepared to provide a minimum of two (2) Loading Site Monitors to be stationed <br /> at each "Control Point." Each truck driver will provide a load ticket that validates where the <br /> material originated and that it is eligible for pickup. Load tickets will be completed in accordance <br /> 01111/ with established procedures and as a minimum contain either a street address or the nearest <br /> intersection to be valid. The volume of debris hauled will be estimated at the debris management <br /> site by the City's Debris Management Site Monitor. <br /> b. The Loading Site Monitors will accurately and legibly complete tickets (electronically and/or <br /> manually) provided by the driver to verify: <br /> • Date <br /> • Preprinted Ticket Number <br /> • Truck Number <br /> • Truck Capacity in Cubic Yards <br /> • Load Size, either in Cubic Yards or Tons (including percent full and certified capacity) <br /> • Truck Driver's Name & Company Name <br /> • Debris Classification: <br /> o Mixed <br /> o Vegetative <br /> o Construction & Demolition Debris <br /> o Other <br /> • Zone/Sector <br /> • Dumpsite Location and Destination <br /> • Loading Time (from work site) <br /> 0 • Loading Site Monitor's Name and Signature <br /> Disaster Program & Operations, Inc.
