Ever rises bc. <br /> QLFUR. p . <br /> DISASTER RESPONSE DIVISION <br /> 4111 Quality Control Officer: The Quality Control Officer is responsible for the direct <br /> supervision of all superintendents and subcontractor and superintendents to ensure that the <br /> work is being performed according to the Contract Plans and Specifications and TFR's <br /> Contractor Quality Control Program. He/she will supervise the Quality Control Personnel <br /> and advise other job personnel in advance of Quality Control Requirements; submittal <br /> material, inspections, etc. of their work. When work does not comply with contract <br /> requirements, he will immediately advise the Project Manager and together they will decide <br /> what corrective action should be taken. The corrective action taken will be shown in the <br /> daily log. Different levels will have different <br /> requirements. All final versions of all forms will be approved with the final CQC Plan, <br /> including any additional required modifications. This discussion shall include the general <br /> procedures set forth below: <br /> • All loading and hauling crews are under the direction of a TFR supervisor. <br /> • Daily reports are maintained by the crew foreman and recoding all equipment down <br /> time for repairs on the daily reports. <br /> • Hours, of each piece of equipment and each employee, are recorded and reconciled <br /> with our client representative daily. <br /> • Daily Reports shall have daily and year-to-date totals for each piece of <br /> equipment/personnel tasked. <br /> • Signatures of both TFR's representative and by a client representative are required <br /> on all Daily Reports. <br /> The TFR Quality Control Officer shall prepare, sign and submit to our client representative <br /> a Quality Control Summary Sheet. This report at a minimum shall include originals and one <br /> copy of all levels of QC reports received and in addition shall include a summary of safety <br /> issues, infrastructure damage, total numbers of trucks loaded, equipment and plant hours <br /> worked and idled or down, testing performed and by whom, loads and quantities hauled to <br /> DMS, quantity of debris reduced, number of subcontractors working, contract non- <br /> compliance issues and all corrective actions. <br /> 57 <br />
