Eurprises,teIRc. <br /> DISASTER RESPONSE DIVISION <br /> ® Damage Control and Reporting <br /> The goal of sound debris removal is to protect the public and private infrastructure and the <br /> overall health of citizens while collecting and disposing of debris rapidly. TFR is fully <br /> committed to this end and strives for a goal of ZERO claims. TFR takes a proactive <br /> approach to the protection of infrastructure. During "Toolbox" Meetings, TFR's Quality <br /> Control Officer shall address the following general procedures to avoid careless activities <br /> during collection: <br /> • Operators are reminded to stay on the rights of way and not to enter private property <br /> during the loading process. <br /> • Operators are cautioned to be observant and conscious of their surroundings always, <br /> in particular being aware of hanging and downed power lines and structures hidden <br /> by debris (such as fire hydrants, water mains, fences, etc.). <br /> • Operators are cautioned about debris weight distribution to be maintained during the <br /> loading process. <br /> • Operators are instructed not to overload trucks with debris in such a manner that the <br /> transporting of the debris could cause the excess debris to become dislodged and fall <br /> into traffic areas or snag low or hanging power lines. <br /> • Debris shall not hang over vehicle sides or extend more than 24 inches above the <br /> sides. Overloaded trucks shall be trimmed or removed prior to departing the loading <br /> area as instructed by the Crew Foreman. <br /> ® However, a few claims are unavoidable. TFR maintains a strict Damage Control and <br /> Reporting Procedures to ensure that each claim is handle with expediency and care while <br /> meeting the needs of the claimant. TFR's Quality Control Officer shall address the Damage <br /> Control and Reporting Procedures with all field ROW personnel during initial safety <br /> meetings. Crew Foreman's are required to report all damages directly to the Quality Control <br /> Officer immediately. Upon notification, the Quality Control Officer will prepare a Damage <br /> Report without delay, and is instructed to seek the following information: <br /> 1. Name, address and phone number of property owner. <br /> 2. Who was present when the accident took place? <br /> 3. Detailed description of what happened, including what activity was being <br /> performed at the time of the damage, why did it happen and documenting <br /> all unsafe conditions, equipment and/or acts. <br /> 4. To the best of the witness's knowledge, was there a previously known <br /> and/or reported problem or condition associated with the accident? <br /> Follow-up: <br /> 1. Date, time and record of TFR's Quality Control personnel that investigated <br /> the damage. <br /> 2. Date, time and record of when and who (property owner) was contacted <br /> and notes as to what was discussed or agreed. <br /> 3. Date and time of repair, or resolution agreement. <br /> 62 <br />
