Eoterprises, lc. <br /> DISASTER RESPONSE DIVISION <br /> Private Property (ROE) Program Procedures: • <br /> The Company's Quality Control Manager shall discuss the requirements and procedures for <br /> the removal of debris from private property under the Right of Entry (ROE) Program with <br /> all Crew Foremen. This discussion shall include the general procedures set forth below: <br /> 1. Before a demolition project on private property can begin, a ROE document must be <br /> executed, including: <br /> a. Authorization to enter the property for demolition but a <br /> b. Hold Harmless Agreement accompanied by a non-duplication of benefits <br /> agreement from the client. <br /> 2. TFR crews are not permitted to enter private property except at the direction of TFR <br /> ROE Manager or a client representative. <br /> 3. Proper completion of a TFR ROE Program Checklist <br /> a. HHW crews will be assigned to remove any hazardous materials from the <br /> property. <br /> b. All HHW materials will be cataloged and disposition defined. <br /> c. All utility connections will be inspected to ensure that they are disconnected <br /> After demolition, all materials can be either placed on the right of way to be picked up under <br /> the debris removal agreement or taken directly to the DMS for separation and disposal. <br /> Rental of Equipment with Operators: <br /> 1. Inspection and Identification of Equipment & Materials before any machinery or <br /> materials are placed in use; they shall be inspected and tested by competent <br /> personnel. The Contractor's personnel will perform such duties. <br /> 2. The contractor's personnel chosen by the Quality Control Officer will perform <br /> inspection of all machinery and materials daily. <br /> 3. Records of inspections shall be maintained at the site and shall be available on <br /> request to the client representative. <br /> 4. Preventative maintenance procedures recommended by the manufacturer shall be <br /> followed. <br /> 5. All repairs on machinery or materials shall be made at a location, which will provide <br /> protection from traffic for repairmen. Any machinery or equipment found by the <br /> contractor to be unsafe would be shut down until unsafe conditions have been <br /> corrected. <br /> 6. Only TFR designated personnel shall operate machinery and mechanized equipment. <br /> 7. The Quality Control Supervisor shall instruct all operators in the use and details of <br /> Safety Operations. <br /> 8. Operators of machinery will establish and confirm requirements daily as to comply <br /> with contract documents with TFR's representative. <br /> 9. Equipment: Material deficiencies observed shall be reported to the proper TFR <br /> personnel for correction. <br /> 10. All safety procedures will be used in accordance with TFR's Accident Prevention <br /> Plan. <br /> ® 11. All phases of work being performed will be inspected daily by the responsible <br /> member of the Quality Control team. <br /> 61 <br />
