(a) Except for applications to collocate small wireless facilities on <br /> existing utility poles in the public rights-of-way, an American Land <br /> Title Association (ALTA) survey or other survey that may be <br /> specified in the City's application form demonstrating that the <br /> proposed location of the facility or utility pole is within the public <br /> rights-of-way, unless waived by the City in its sole discretion, <br /> pursuant to information confirming the proposed facility is located <br /> with the public rights-of-way: <br /> (b) The type of proposed facility. location of the proposed facility, and <br /> the dimensions, height, footprint, stealth design, and concealment <br /> features of the proposed facility: <br /> (c) The Global Positioning System (GPS) coordinates of the proposed <br /> facility. The GPS coordinates shall be based on the reading from a <br /> handheld mobile GPS unit set to Datum NAD 83 or WGS84. GPS <br /> coordinates based on Google Earth or similar application may be used <br /> where areas of shading occur due to overhead canopy. GPS <br /> Coordinates shall be provided in decimal degrees at a six (6) decimal <br /> point precision; <br /> (d) Whether the proposed facility is proposed within a location subject to <br /> restrictions pursuant to this Ordinance; <br /> (e) Distances between the proposed facility and the edge of nearby <br /> pavement, sidewalks. driveways. ramps, the nearest residential <br /> properties, nearby drainage systems, trees. ground-mounted <br /> equipment, nearby structures in the public rights-of-way, underground <br /> utilities and other above-grade and below-grade structures and <br /> utilities located within the public rights-of-way: <br /> (f) For new communications facilities, a sketch showing pavement. <br /> sidewalks, driveways, ramps. trees. below-grade utilities, and other <br /> above-grade and below-grade structures and facilities within the <br /> public rights-of-way located within a fifty (50) foot radius; <br /> (g) Sufficient specificity demonstrating compliance with applicable <br /> codes. including the Florida Building Code. most current edition. <br /> specifically including but not limited to terms of compliance with the <br /> High Velocity Zone Criteria specified therein, the 2010 Florida <br /> Department of Transportation Utility Accommodation Manual, as <br /> applicable: National Electrical Code, the National Electrical Safety <br /> Code and the "Safety Rules for the Installation and Maintenance of <br /> Electrical Supply and Communication Lines" established by the <br /> Department of Commerce, Bureau of Standards of the United States. <br /> as may be amended: <br /> (h) The routes of all new transmission and distribution lines to be placed <br /> or maintained in the public rights-of-way in connection with the <br /> proposed facility (such lines may be subject to separate permit <br /> requirements) or any connection or tie-in to any existing lines in the <br /> public right-of-way, as may be applicable based on the proposed <br /> installation: <br /> (i) Certification that the proposed facility will not materially interfere <br /> 41 <br /> City of Sunny Isles Beach CROW Ordinance 10-17-18 <br />
