City of Sunny Isles Beach | RFQ No. 20-07-01 | Continuing Professional Consulting Services (CCNA) <br />Landscape Inspection (If needed): Before construction site <br />inspection to see if any trees / palms will be impacted and any <br />root pruning must be done. Meeting with County Landscape <br />Inspector on expectations. During construction, take soil <br />samples, nursery visit with contractor to tag plant material, <br />Florida Grade #1, or better to ensure minimum requirements <br />met. Ensure shop drawing submittal of fertilizer, staking/ <br />guying, soil, pre-emergent herbicide, mulch and sod. <br />Document Control and Warranty: Our personnel will strive <br />to maintain legible, complete, properly indexed/archived, and <br />retrievable documents. We are committed to utilize the latest <br />software, our staff will also work with your in-house staff to <br />ensure that all warranty information is collected; tracked and <br />periodic inspections conducted to ensure the County received <br />the full value of these improvements. The incorporation of the <br />value added specification stipulates the inspection frequency <br />for each incorporated element and we will ensure that all <br />relevant information is entered into the County’s database <br />system. Our team will coordinate all the warranty requests <br />between the County and the contractor, we will perform <br />the periodic inspections during the life of our contract to <br />determine the corrective action for each deficiency, and we <br />will notify the contractor in writing of the items covered by the <br />warranty that require corrections. <br />Resident Compliance Support (RCS): We will monitor and <br />maintain files to comply with the Federal and State rules <br />and regulations concerning the compliance program. We <br />will maintain our constant lines of communications with the <br />County or FDOT resident compliance manager to ensure the <br />expedited flow of information between the team, County, <br />and FDOT. We will conduct a pre EEO/OJT/DBE meeting <br />with contractor and County or FDOT representative to <br />revive the compliance requirements listed in the contract. <br />Including Davis Bacon Act, payroll format, classification <br />and pay information, interviews with the contractor’s staff <br />to verify that the information provided in the payrolls are <br />accurate. Non- compliance letters will be issued as needed <br />for any compliance violation. Our RCS will monitor the on- <br />the-job training (OJT) program for compliance with the EEO <br />handbook and contract requirements. <br />During Substantial Completion Walk-through: 30 Days <br />prior to completion of project, we will meet with Contractor, <br />County Project Manager and our staff and walk entire site <br />to make note of deficiency’s. We will then generate a punch <br />list excel sheet. As each item is completed and/or fixed, <br />our inspector will date and comment on the completion of <br />the item. After all items are complete and confirmed by our <br />inspection staff prior to last day of contract, we will have a <br />final walk-through with the County Project Manager, CEI staff <br />(PA, SPE, inspection staff), FPL (and/or other designated <br />utility company), City (if needed) and Contractor. <br />Quality Assurance (QA): CGA will incorporate our QA <br />Checklist with the current Final Estimate Submittal checklist <br />Performs thorough review and oversight of the Quality Control <br />(QC) density log book by verification personnel to minimize <br />errors and potential delay to the final material certification. <br />• Perform bi-weekly reviews of material testing <br />results to ensure resolution of open samples, failing <br />test results and missing QC and/or verification <br />information. <br />• Generate and submit an accurate and timely Final <br />Offer Letter within 30 days of Final Acceptance. <br />• Maintain current and accurate records of field <br />changes and plan revisions for the preparation of the <br />as-built plans. <br />• Ensure that Maintenance of Traffic Plans are in <br />accordance with the MUTCD and that one lane of <br />through traffic is maintained throughout the whole <br />project limits. <br />• Continue to keep the residents, hospital, school and <br />businesses of the immediate area apprised of all <br />construction phasing throughout project duration. <br />18
