STANDARD FORM 330 (REV. 8/2016) PAGE 21 <br />RESPONSE TO REQUEST FOR QUALIFICATIONS FOR <br />Continuing Professional Consulting Engineering Services-Environmental Engineering <br />#BID 20-07-01 <br /> <br /> <br />Building Inspection and Survey- <br />Buildings are inspected for the presence of suspect asbestos -containing materials during the site inspection. Each <br />observed material is described, photographed, measured and sampling is taken using procedures outlined in 40 CFR 763 <br />and F.S. 255.551-255.565. If plans of the building are not available, GBTS develops a schematic flo or plan for use in <br />describing location and extent of asbestos-containing materials. Each suspect material is assessed by the inspector for <br />its condition, friability and hazard potential. Inaccessible areas are evaluated on a case-by-case basis. The inspector will <br />determine if minor demolition is required to access areas such as pipe chases, hard ceilings, or walls to determine the <br />presence of asbestos-containing materials behind these structures. <br />Survey and Assessment Report- <br />After a site survey has been performed and samples of suspect materials are analyzed, a survey report is generated. The <br />report will contain a summary of the results of the survey, classification of asbestos-containing materials by NESHAP <br />category, recommendations for response actions, and cost estimates. Each report will include all documentation <br />produced for the survey, including: site plans, bulk sample numbers and locations, locations of -asbestos-containing <br />materials, photographs of each homogeneous area ad bulk sampling location, and quantities and condition of asbestos - <br />containing materials. <br />Plans and Specifications- <br />For each surveyed building, an Operations and Maintenance Plan and / or Abatement Specification will be provided for <br />use by the Client. Operations and Maintenance Plans reflect the fact that these facilities will be demolished in the near <br />future. Abatement specifications will be prepared as a Master Project Manual, which encompasses all procedures <br />currently used in asbestos abatement. For each building, a specific work p lan is provided which addresses the site- <br />specific conditions. <br />Contract Administration- <br />As required, GBTS will assist our client with all aspects of Bidding from invitation to award. This phase of the work is <br />especially important in obtaining a fair cost proposal from the abatement contractors and eliminating future change <br />orders arising from poor understanding of the nature of work. The performance of the consultant during the pre -bid <br />stage is crucial to resolving any significant difficulties. <br />During this period, GBTS will provide all clarifications to bidding contractors to resolve any technical problems and <br />provide satisfactory answers to questions from contractors. Every effort will be made to assure the contractors <br />understand the scope of work and requirements for the project. <br />Project Supervision/Air Monitoring- <br />GBTS will provide a project supervisor for all abatement / demolition work to be performed through the Client. The <br />project monitor is responsible for coordinating and documenting activities during asbestos abatement, including air <br />monitoring. The project supervisor will remain onsite during all abatement activities and will assure the contractor is <br />performing work in compliance with all federal, state, and local regulations. The project sup ervisor will perform all <br />visual inspections, onsite PCM analysis and final air testing. Final air testing results will be made available to the Client <br />immediately upon verbal confirmation from the laboratory. <br />Final Clearance Report- <br />After completion of the asbestos abatement project, or per purchase order, GBTS will issue a final clearance report to <br />the Client. The report will contain all documentation of the asbestos abatement project including; available project <br />design and history, completed project specifications and work plans, air monitoring results, contractor submittals, daily <br />logs and certification letters.
