BRIEF DESCRIPTION OF PROJECT AND RELEVANCE TO THIS CONTRACT (Include scope, size, and cost) <br />Underwater Engineering Services, Inc. (UESI), UESI performed as a subconsultant to Ayres Associates and provided professional <br />engineering inspection services for underwater structural assessments of three movable bridges located in and north of Daytona Beach, <br />FL. The project also included performing underwater quality assurance inspections of three bridges. UESI provided a three-person dive <br />team. Two of the movable bridges were led and inspected by a registered Florida Professional Engineer-diver. The third movable bridge <br />was led and inspected by a Florida Certified Bridge Inspector. The scope included all underwater elements, including piles, piers, footings <br />and fender systems. A report was prepared for each structure which included photographs, findings, recommendations and element <br />checklists and ratings similar to that used by the Florida DOT. The quality assurance inspections were done by a three-person dive team <br />led by a Florida Certified Bridge Inspector. The deficiency notes from the regular underwater bridge inspections were reviewed and verified <br />by the QA team. The contract amount was $9K <br />This project had underwater and topside condition assessments of bridges. Ratings followed the system used by FHWA and FDOT. <br />Team experience: <br />Mr. O’Connor was the Project Manager and performed the inspections of two of the movable bridges. Mr. Faulk was dive supervisor and <br />performed the inspections of one movable bridge and the three QA bridges. <br /> <br />25.
