<br />City of Sunny Isles Beach |Request for Proposal No. 22-09-01 36 <br /> <br />statement. In lieu of audited financial statements, proposers may submit other evidence, <br />acceptable to the City, of financial responsibility. Such evidence may include but is not <br />limited to letter(s) of credit with a financial institution indicating the proposer's line of <br />credit and the level of financing the institution will offer the proposer for capital <br />procurement or certified copy(ies) of federal income tax return(s), and unaudited <br />financial statements. <br /> <br />6. REFERENCE(S) - PER MQR # 2 <br />Firm shall provide a minimum of one (1) reference letter from a landlord in which <br />Contractor served as Primary tenant for a restaurant facility similar in size. OR, Proposer <br />must provide a good standing bank reference reflecting their mortgage payment history <br />within the last five years. <br />• The Reference must be supplied on bank/landlord holder’s Letterhead <br />• The Reference letter must be signed with contact person and phone number. <br /> <br />The City retains the right to request any additional information pertaining to the <br />Proposer's ability, qualifications, and procedures used to accomplish all work under the <br />contract as it deems necessary to ensure safe and satisfactory work. <br /> <br />7. PROPOSED RENTAL SCHEDULE <br />Each Proposer shall submit the chart below in their price proposal. Any pricing conditions <br />or contingencies must be clearly stated. <br /> <br />8. CONTRACT FORMS <br />All Proposers must complete and submit Bid Submittal Forms and Affidavits included in <br />this document. <br /> <br />9. EXCEPTIONS: <br /> Firm must list any exceptions taken to the terms and condition in this RFP. <br /> <br /> <br /> <br /> <br />END OF SECTION <br />
