The plans will be prepared in CAD format using <br />industry standards for this type of construction. Cummins Cederberg will provide structural <br />calculations for the design and subsequent building permit processing as appropriate. The plans <br />and calculations will be signed and sealed by a Florida registered Professional Engineer. The <br />following drawing sheets are anticipated: <br />■ Cover Sheet - Location maps, titles <br />■ Site Plan - Existing conditions, survey, water depths <br />■ Proposed Plan - Seawall layout, piling locations, turbidity curtains <br />• Sections - Cross sections, structural details <br />■ General Notes - Specifications, materials, list of abbreviations <br />Deliverables: Signed/sealed construction plans (11x17 or 22x34) <br />Signed(sea/ed structural calculations <br />Fees for Task 1 are lump sum (including expenses): .................................................. $18,400.00 <br />Task 2 - Environmental Permitting <br />Task 2.1- Miami -Dade County DERV! Class I Application and Processing: <br />Cummins Cederberg will prepare and process a Miami -Dade County Division of Environmental <br />Resource Management (DERM) Short Form Class I Coastal Construction Permit Application, <br />requesting authorization of the proposed seawall, pursuant to Chapter 24 of the Miami -Dade <br />County Code. Cummins Cederberg will maintain contact with DERM staff to review plans and <br />provide additional information. The Client will provide Cummins Cederberg with a signed <br />application form, contractor information, performance and mitigation bonds (if required), <br />application fees, and permit fees (to be determined based on estimated construction cost). <br />Task 2.2 SWERP Exemption Verification f=orm and Processing: <br />Cummins Cederberg will prepare and process a Statewide Environmental Resource Permit <br />(SWERP) Exemption Verification Form requesting authorization for the proposed seawall <br />installation, pursuant to Chapter 62-330.051, F.A.C. Cummins Cederberg will maintain contact <br />with FDEP to review plans and provide additional information. The Client will provide Cummins <br />Cederberg with an exemption fee in the amount of $100.00. <br />Task 2.3 USACE Application and Processing: <br />Cummins Cederberg will prepare and process a Department of the Army Application with the <br />USACE, requesting verification of the Nationwide 13 Permit for the proposed seawall. Cummins <br />Cederberg will maintain contact with the USACE to review plans and provide additional <br />information (e.g., JaXBO forms, construction methodology).
