?y�'Py� t�Q• yiyr <br />�`r�' QF svK Ada <br />City of Sunny Isles Beach <br />18070 Collins Avenue <br />Sunny Isles Beach, Florida 33160 <br />(305) 947-0606 City Hall <br />(305) 949-3113 Fax <br />MEMORANDUM <br />TO: Honorable Mayor and City Commissioners <br />FROM: Stan Morris, City Manager <br />DATE: May 18, 2023 <br />RE: Approving Change Order No. 6 with The Stout Group for the Golden Shore <br />Utility Undergrounding Project <br />RECOMMENDATION: <br />It is recommended that the City Commission approve this proposed resolution in support of increasing <br />the spending authority for The Stout Group referenced in CO No. 6 due to unforeseen condition! <br />relating to additional conduit required for Atlantic Broadband, and FPL, depth of existing asphalt <br />paving, increase to contingency for multiple issues in the Golden Shores neighborhood. <br />REASONS: <br />Change Order No. 6 increases the overall contract amount by $426,783.00 as noted above. <br />Atlantic Broadband (now Breezeline) design coordination failed to adequately address original <br />conversion of utility matters between the Collins Ave. phase and the Golden Shores project an <br />additional 11,970 linear feet of conduit is required to complete the interface and provide service to <br />the neighborhood. <br />FPL documents omitted the numerous residential service connections between new transformers and <br />individual hand holes located in the public Right of Way, along with Feeder conduit connecting with <br />Dumbfounding Bay, for this approximately 6,500 linear feet of conduit is required. <br />Ongoing unforeseen existing utility conflicts impact the timely and uninterrupted installation of new <br />equipment including communication lines, pumpstation piping, and gas/sewer line, these issues are <br />dealt with as contingency expenses resulting in an increase to the line item <br />The original contract documents anticipated the depth of the existing asphalt paving to be 4" in <br />Item Number: 10.17 <br />
