Employee picture identification matching names on records. Picture IDs <br />can be photo copies of training cards; however, pictures of employee <br />faces must be viewable vs. dark images from poor quality photo copying. <br /> <br />3. Subcontractor License: Submit proof of license for asbestos-related contracting <br />from the Florida Contractors State License Board (CSLB) as required by Florida <br />Business and Professions Code, Section 7058.5 et.seq. <br /> <br />4. Subcontractor Registration: Submit proof of registration with the Florida Division <br />of Occupational Safety and Health (DOSH) for asbestos-related work as required <br />by Title 8 CCR, Sections 341.6 to 341.9. <br /> <br />5. Carcinogen Registration: Submit proof of carcinogen registration (report of use) <br />with DOSH as required by Florida Labor Code, Section 9000 et. seq. <br />6. Bay Area Air Quality Management District (BAAQMD) Notification: Submit proof <br />of notification made to the BAAQMD, 10 working days prior to asbestos <br />demolition or renovation activities, as required by BAAQMD Regulation 11, Rule <br />2. Subcontractor shall update and resubmit notifications as needed due to <br />changes in the project schedule or material quantities. The subcontractor shall <br />be responsible for obtaining variances from the BAAQMD as necessary. <br /> <br />7. Respiratory Protection Program: Submit company’s Respiratory Protection <br />Program. <br /> <br />8. Written record from negative air machine and HEPA vacuums cleaner challenge <br />aerosol testing on the day of the testing. <br /> <br />9. Per 29 CFR 1926.1101 (f)(5), the subcontractor shall notify their employees of <br />exposure monitoring results. LBNL requires written documentation from the <br />subcontractor that demonstrates the subcontractor’s employees were notified of <br />their results within 5 days of receiving the results including both excursion and <br />calculated 8-hour TWA results for all results, not just those results above the PEL <br />or excursion limit. <br /> <br />D. Closeout / Ongoing Project Submittals <br /> <br />1. Personal air sampling results collected by the asbestos abatement subcontractor <br />for airborne asbestos within 24 hours of sample collection. <br /> <br />2. Pressure differential recorder readings shall be submitted in written form (i.e., <br />circular chart or other form of print out). <br /> <br />E. All submittals will be provided to LBNL three (3) working days prior to mobilization for <br />each individual site job and must be approved prior to work commencing. <br /> <br />1.4 QUALITY ASSURANCE <br /> <br />A. Qualifications: <br /> <br />1. Qualifications of Subcontractor: <br /> <br />a. Work performed under this Section shall be by a single Subcontractor.
