LUNRCO <br />• COVID-19 Safety and Reporting Procedures. Lunacon developed a Corporate COVID-19 <br />response plan to protect workers, which follows OSHA, CDC Interim Guidance for <br />Businesses and Employers, and the Department of Defense (DoD) guidelines. All job site <br />boards include this important information. COVID-19 is addressed in all APPS submitted to <br />the government. During daily safety meetings, the safety officer addresses COVID-19, and <br />all workers on site must answer questions on how they feel It is stressed to everyone during <br />this meeting that if someone feels ill, they must leave and stay home. Each site has hand <br />sanitizer and antibacterial soap to support all personnel. If required, wash stations are also <br />provided with port -a -johns on the project site. <br />* First Aid Treatment Plan. Regardless of how detailed our construction company's safety <br />plan is and despite how in-depth and meticulous ourtraining program is, accidents involving <br />injuries can still occur. When a worker suffers an injury, it is vital to have a first aid program <br />in place. Because no two construction sites are the same, Lunacon designs a job site- <br />specific first aid program for each project so that our workers can receive the best possible <br />care should they suffer an injury. The procedures for developing a job site-specific first aid <br />program is part of our overall safety plan. Our first aid program is available to workers who <br />keep them on hand for reference. On each project, the assigned SSHO and Superintendent, <br />at a minimum, are the onsite qualified and certified First Aid providers. The assigned first aid <br />providers receive training that includes basic first aid, CPR, and an automated external <br />defibrillator (AED). Additional training may be required depending on the type of work being <br />performed at the job site. Lunacon ensures all retraining is done yearlyfor CPR, using an AED, <br />and every 2— 3 years for first aid training, depending on how long the certification is good for. <br />In addition, the site safety plan includes all emergency numbers, local hospital information, <br />and directions. All projects have extensive safety kits that are checked daily and restocked. <br />• Workplace incident reporting procedures. The Site Safety and Health Officer will report all <br />accidents (up to 24 hours afterward) to the Lunacon executive team and . The Site Safety and <br />Health Officer will also perform thorough accident investigations and complete accident <br />report forms using Contractor Significant Injury Reports to be transmitted to the as soon as <br />possible (but at most five working days following the incident). The Site Safety and Health <br />Officer will coordinate accident investigations and reporting activities to maintain the OSHA <br />recordable and other applicable injury logs and implement corrective actions as soon as <br />reasonably possible. For immediate notification of major accidents, the Site Safety and <br />Health Officer will immediately notify the Lunacon Executive Team and and follow up this <br />notification with written reports within four (4) hours of the occurrence. Major accidents <br />include: <br />o A fatal injury or permanent partial or total disability <br />o The hospitalization of three or more people results from a single occurrence. <br />o Property damage more than $100,000 <br />• Subcontractor Safety Program Compliance Requirements. Lunacon considers our <br />subcontractors valuable members of our project teams and encourages active involvement <br />and communication from our subcontractors at every step.
