The form is available through the FAA website: https://oeaaa.faa.gov where the contractor <br />may "e-file" the information. This office will also need to review and approve the construction <br />heights and locations associated with this construction project, if the crane heights exceeding 200 <br />feet Above Ground Level (AGL). You may file by completing the "MDAD Permissible Crane <br />Height Determination Form" available through this office or our website, http://www.miarni- <br />airport.com/planning_forms_maps.asp. <br />In accordance with FAA requirements, it is required that FAA Form 7460-2, Notice of Actual <br />Construction or Alteration, be e-filed any time the project is abandoned and at least 10 days prior to <br />the start of construction (7460-2, Part 1) and within 5 days after the construction reaches its greatest <br />height (7460, Part 2). Upon completion of this project, MDAD's Aviation Planning, Land -Use and <br />Grants Division must receive a copy of the surveyor's maximum height certificate. No Certificate <br />of Use or Certificate of Occupancy shall be issued by a municipal building official or Miami -Dade <br />County until approval is obtained from this office after certifying that the structure was built no <br />higher than the height approved by this letter. The approval shall be issued by this office after <br />submission of the required information including as -built elevations certified and prepared signed <br />and sealed by a State of Florida licensed surveyor, architect or engineer. Such elevation as -built <br />certification shall be 8.5" x 11" in size and contain an elevation view of as -built certification with <br />
