V <br />LO <br />Ms. Karly Beth Van Dam <br />December 11, 2025 <br />Page 3 <br />Latitude and Longitude Coordinates in NAD 83 (in degrees, minutes, seconds format with at least 2 <br />decimal places accuracy for the seconds number) noted for the height of the structure and the height <br />of any appurtenances. The height shall also be indicated for the point closest to the airport. <br />The analysis in this letter is based, in part, on the information provided by Ms. Karly Beth Van <br />Dam of Capitol Airspace Group. The architectural elevations and site plan dated September 9, <br />2025, signed by Sandy Lee Peaceman, R.A. of Cohen, Freedman, Encinosa & Associates. The <br />Survey, dated April 25, 2025, signed by Daniel C. Fortin Jr., P.M.S., of Fortin, Leavy, Skiles, Inc. <br />Which includes specific building and site locations and heights. Any changes in building <br />locations/layouts or heights will void this analysis. Any future construction or alteration, including <br />an increase to heights requires separate notice to the FAA and MDAD. <br />This. determination expires one year from the date of issuance unless extended, revised, or <br />terminated by MDAD's Aviation Planning, Land -Use and Grants Division. Any requests for an <br />extension of the effective period of this determination must be submitted by -the applicant prior to <br />the expiration date. At such time, staff will re-evaluate the application or structure to determine if <br />any significant changes to the structure and/or to the review criteria have occurred. MDAD's <br />Aviation Planning, Land -Use and Grants Division will then decide if the determination in this letter <br />may be eligible for an extension of the effective period. <br />This determination does not constitute MDAD approval or disapproval of the physical development <br />or use involved in the proposal.
