We are entirely confident that these project descriptions illustrate that we not only meet, but exceed, the requirements listed in both the <br />Scope of Services and MQRs for all disciplines upon which we are bidding. Here is the order in which our Project List is laid out: <br />• Below, we present a list of projects to demonstrate we have experience working with the regulatory agencies listed in the MQRs. <br />• Starting on the following page, we present general data sheets about some of our most relevant, long-term clients. <br />• Following the general data sheets, you will find sheets that depict specific projects for relevant clients. <br />Experience Working with Florida Regulatory Agencies <br />CGA has provided numerous projects permitted through organizations listed in the RFQ. Specific projects and other details are listed below. <br />Miami-Dade County Department of Transportation and Public Works (DTPW), which includes the Traffic Engineering Division and the <br />Highway Construction and Engineering Division <br />• Miami-Dade County, FL - Underline Phase 3 - separate contracts awarded by DTPW (1. Landscape Review and Arborist Services and 2. <br />CEI Services) <br />• City of Miami Beach - Pedestrian Priority Zone Project on Lincoln Road - permitting through DTPW <br />Florida Department of Transportation (FDOT) <br />Several of our proposed CEI team members have valid FDOT certifications, including Erik Padron, PE, MSCE (CGA’s proposed CEI Team Lead). <br />CGA’s clients include FDOT Districts 1, 4, 6 & 7. There are descriptions of representative FDOT projects throughout the proposal, including in the <br />general data sheets and specific project sheets on the following pages. Here are some additional projects not included as full sheets: <br />• FDOT District 4: I-75 Alligator Alley Environmental Education Project <br />• FDOT District 4: SR-848 Stirling Road from W Of N 58th Ave to US-1 <br />• FDOT District 6: General Assigned Contract, including CA630; TWO #5; Project FM #445958-1-62-01 <br />Department for Environmental Resources Management (DERM) <br />• Bal Harbour Village, FL - Multiple Water Main and Sewer Improvements - DERM permit <br />• Bal Harbour Village, FL - Yearly GIS Data Submittal to DERM <br />• Village of Palmetto Bay, FL - Sub-Basin 57/96 - DERM permit <br />Florida Department of Environmental Protection (FDEP) <br />Nicholas Kanelidis, PE (CGA’s proposed General Civil Engineers Team Lead) is a licensed FDEP Stormwater, Erosion & Sedimentation Control <br />Inspector (No. 40536). Additionally, CGA has FDEP experience from the following projects: <br />• Bal Harbour Village, FL - Ongoing Professional Services - involves FDEP Annual Reporting for NPDES <br />• Oleta River State Park in North Miami, FL - Continuing services FDEP contract for septic tank conversion project on these state-owned lands <br />• Zachary Taylor State Park in Key West, FL - Overflow Parking Lot - permitted project through FDEP and SFWMD <br />• City of Miami Beach - Middle Beach Recreational Corridor - FDEP Coastal Construction Control Line Permitting <br />• City of Hallandale Beach, FL - Three Islands Reuse Water Pipeline - FDEP and the Broward County Environmental Resource Department <br />permits were obtained for these improvements <br />• City of Hallandale Beach, FL - Raw Water Pipeline - Permitting to FDEP and Broward County Highway Construction and Engineering Division <br />(BCHCED) approved before project went out to bid <br />• Town of Davie, FL - SW 30th St Reuse Water Main - Permitted through FDEP, Broward County, and Town of Davie Engineering Department <br />• City of Wilton Manors, FL - NW 3rd Avenue Area Water Main Replacement, Phase I - Permitting responsibilities include coordination with <br />FDEP, BCHCED, and Broward County Traffic Engineering Division <br />Army Corps of Engineers (ACOE) <br />CGA’s Otto Letzelter CFM, PE, SE, SI, BCA, SFPE, CFPS received Construction Management for Contractors Certificate from ACOE.
