Proposed Free Standing Emergency Room (FSER) <br />6506 Okeechobee Blvd., West Palm Beach, FL <br />The proposed project includes construcƟon of a <br />10,860 square-foot, one-story, free-standing building <br />with a drive-thru canopy. <br />Geotechnical Engineering Study <br />PSI completed a field exploraƟon and geotechnical <br />evaluaƟon. The purpose of the study was to evaluate <br />the subsurface condiƟons at the site and develop <br />geotechnical engineering recommendaƟons and <br />guidelines for use in preparing the design and other <br />related construcƟon documents associated with <br />structure foundaƟons. PSI’s scope of services included <br />drilling soil borings, performing representaƟve <br />laboratory tesƟng, and preparing the geotechnical <br />engineering report. <br />Our final report briefly outlined the available project <br />informaƟon, described the site and subsurface <br />condiƟons, and presented the recommendaƟons regarding the results of seven Standard PenetraƟon Test (SPT) <br />soil borings drilled to depths of 10 to 20 feet below exisƟng grades within the proposed FSER building and <br />associated pavement areas. The Geotechnical Engineer warranted that the findings, recommendaƟons, <br />specificaƟons, or professional advice contained were made in accordance with generally accepted professional <br />Geotechnical Engineering pracƟces in the local area. <br />Dates: 03/2022-05/2024 | PSI Fees: $6,950 | PSI Project 03971860 <br />Phase I Environmental Site Assessment Services <br />The subject property consists of an approximate 0.11-acre land area that is planned for the development of an <br />approximate 11,250 square feet single-story medical facility. The Phase I ESA was performed in general <br />accordance with ASTM E1527-21 Standard PracƟce for Environmental Site Assessments: Phase I Environmental <br />Site Assessment Process (ASTM E1527-21). PSI performed the assessment under the supervision of an <br />environmental professional (EP) as defined in 40 Code of Federal RegulaƟons (CFR) 312.10. <br />The scope of services include: <br />•Regulatory records review, <br />•Historical records review, <br />•Site reconnaissance, <br />•Interviews, <br />•Vapor Encroachment Screen (VES) in general accordance with ASTM E2600-22, and <br />•PreparaƟon of a wriƩen report <br />PSI prepared a report of our findings and provided an electronic (.pdf format) copy. One hardcopy can be <br />provided upon request.
