Project Manager. F.R. Aleman provided <br />as-needed surveying, mapping, and SUE support to District 4 on this 5-year districtwide contract ($5M). Under this contract, <br />Ms. Tolstoy was responsible in providing miscellaneous surveying services throughout Broward, Palm Beach, Martin, Indian <br />River, and St. Lucie Counties. The services provided included LIDAR, field surveys, historical baseline and existing right-of- <br />way determination, GPS Surveys, topographic surveys and DTM, general land and aerial photography survey support, <br />monumentation surveys, bathymetric surveys, utility designation and excavation including GIS files, tree surveys, maintenance <br />of traffic, right-of-way control survey maps, right-of-way maps, maintenance maps, right-of-way monumentation maps, <br />boundary surveys, quality assurance reviews, title search plotting maps, sketches, parcel staking, and legal descriptions. More <br />than 60 WOs were successfully provided for about $3.4M up to today. (FDOT Contact: Roberto Chavez, PSM; Telephone: <br />954-777-4574; Start/End Date: 06/2018-06/2021). <br />C9J67 General Engineering Consultant Services - Right-of-Way Modernization Program, FDOT District 6. Project <br />Manager. F.R. Aleman provided surveying services on this 8.5-mile-long project in Miami-Dade County. Ms. Tolstoy was <br />responsible for the planning and execution of the PNC/Control Survey, alignment analysis and calculation, right-of-way mapping <br />and right-of-way monumentation. The projects included were SR 94 from SR 997 to SW 152 Avenue (2.4 Miles); SR 90 from <br />SW 27th Ave to SR5 (3.07 Miles); SR 7 from SR 934 to NW 117 St (2.98 Miles). Under this contract, Ms. Tolstoy also managed <br />design surveys projects, including the establishment of baselines, benchmarks, cross sections, topographical features, DTM, <br />GPS. Mobile LiDAR was used to scan the design corridors, collecting one million points of data per second and photographic <br />images, and creating a 3D point cloud. (FDOT Contact: X A. Negrin, PSM; Telephone: 305-470-5242; Start/End Date: 06/2017- <br />07/2020). <br />C9F94 DW Construction Engineering Inspection Consultant Surveys, FDOT District 6. Senior Project Surveyor. The <br />purpose of this multi-task CEI contract was to provide services to verify both existing pre-construction and post-construction <br />conditions, to ensure design criteria was met by the contractors. As a Prime consultant, F.R. Aleman provided survey and <br />mapping support to the District. This task work order contract involved field surveys for construction projects, including re- <br />establishment of survey baselines, control points, benchmarks, drainage surveys and bridge data, original and final cross - <br />sections, and mast arm bearings, among other scopes. (FDOT Contact: Isaac Wynn, FCCM; Telephone: 305-640-7438; <br />Start/End Date: 04/2016-02/2019). <br />CA603 Mobility Improvements at Dykes Road; 196th Avenue and NW 10th Street, FDOT District 4. Senior Surveyor. <br />The project consisted of 6 Miles of milling, resurfacing, and restriping two existing 12-feet lanes and 4-feet paved shoulder to <br />provide two 11-feet lanes and a 4-feet bike lane (with 2-feet buffer) in both directions along an off-system road. The project <br />also consisted of providing appropriate signing according to MUTCD. Services provided were Horizontal & Vertical control, <br />XYZ for 140 aerial targets, 3D Topographic and DTM along obscure areas, cross sections check, collect drainage structures, <br />RW and Baseline analysis, 45 VVHs including the verified utility table. (FDOT Contact: Roberto Chavez, PSM; Telephone: <br />954-777-4574; Start/End Date: 11/2019-03/2021). <br />Design Services for SR 15 from S of Morgan Road to S of Shirley Drive Project, FDOT District 4. Right-of-Way Specialist/ <br />Survey Leader. Ms. Tolstoy served as the Right-of-Way Specialist and Survey Leader for this complete design survey that <br />combined ground survey with aerial photography. Survey included recovering the historical baseline and determining the right- <br />of-way lines along the entire corridor. 35 Vacuum Excavation Test Holes (VVHs) were performed to confirm the Horizontal and <br />Vertical Locations of underground utilities in conflict with proposed improvements in Pahokee, Palm Beach County. Survey of <br />VVHs targets, including table of verified utilities, were prepared and provided under F.R. Aleman. (FDOT Contact: Jeffrey D. <br />Smith, PSM; Telephone: 954-777-4560; Start/End Date: 10/2017-08/2018). <br />Lyons Road from S of C-14 Canal to Sawgrass Expressway, FDOT District 4. Project Surveyor. This project involved an <br />off-system MPO bicycle and sidewalk 5-mile mobility project that required the widening of the existing pavement between 4 <br />and 7 feet to add bike lanes. Ms. Tolstoy was the Project Surveyor and led the recovery of the historical baseline of survey <br />and right-of-way line for this design project. The scope included horizontal and vertical control, recovering the historical <br />baseline of survey, preparing PNC sheets, ground survey support for the topographic/DTM survey, and providing underground <br />utility survey.
