Page 96CITY OF SUNNY ISLES BEACH • RFQ #26-07-01 • CONTINUING PROFESSIONAL <br />ARCHITECTURAL AND ENGINEERING CONSULTING SERVICES (CCNA) <br />INDOT Vincennes District On-call Scoping - Managed the preparation of the Engineering Assessments which include planning, capacity analysis, safety analysis and other items as required. The following is a list of recent assignments. <br /> SR 57 from Old US 50 to N.C.L. in Washington and Daviess County (PK 60287) – Final Report 11/29/23 <br /> SR 62 TWLTL – Mount Vernon, west side (PK60174) – Final Report 11/2/21 <br /> US 41 Intersections at Petersburg Road, SR 57, and St. George Road (PK60023) – Final Report 12/14/21 <br /> US 231 in Loogootee (PK 60320) – Final Report 10/31/21 <br /> SR 62 at Green River Road in Evansville (PK 60178) – Final Report 10/7/22 <br />INDOT Central Office Safety and Capacity Analysis - Managed the preparation of Traffic Capacity and Operations and Safety Analysis Reports which include planning, capacity analysis, safety analysis and other items as required. Corradino has held this on-call contract on three separate occasions in the last decade. The following is a list of recent assignments. <br /> US 41 at CR 50S/Richland in Gibson County – Final Report 2/25/20 <br /> SR 64 at Hall St. in Gibson County – Final Report 2/24/20 <br /> SR 56 at Slab Rd. (CR 100E) in Scott County – Final Report 3/8/20 <br /> SR 64 at Corydon-Ramsey Rd. in Harrison County – Final Report 3/8/20 <br /> SR 46 at College Mall Road in Monroe County – Final Report 3/8/20 <br /> SR 142 at Baltimore Road in Morgan County – Final Report 2/28/20 <br /> US 231 at CR 750N in Owen County – Final Report 2/28/20 <br /> US 40 at CR 200W in Hendricks County – Final Report 2/28/20 <br /> SR 42 at CR 1200N in Morgan County – Final Report 2/28/20 <br />I-69 at 106th Street New Interchange; Fishers, Indiana - Managed the traffic modeling, environmental services (Environmental Assessment, Approved 11/4/15 and permitting) and prepared the Interstate Access Document (IAD), Final Report 12/5/15. This project was completed with an accelerated schedule. <br />I-65 Added Travel Lanes from US 30 to SR 10; Lake and Newton Counties, Indiana - Provided environmental planning services. He worked with those preparing the Categorical Exclusion for the project. Most importantly, he coordinated with resource agencies and local officials to secure the required permits. This was critical at the Kankakee River crossing.This project involved the reconstruction and widening of the I-65 bridges over the Kankakee River, resulting in the need for Section 4(f) De Minimis approval for the temporary closure of the Levee Trail in the Grand Kankakee Marsh Park. The Grand Kankakee Marsh Park is located along the north bank of the Kankakee River, with property east of and west of I-65. The Levee Trail is an approximately 8-foot wide path situated on the existing levee along the north bank of the Kankakee River. The Levee Trail provides pedestrian, equestrian, and off-road access to the east and west portions of the Grand Kankakee Marsh Park through the northernmost spans of the 3-spanned northbound and southbound I-65 bridges over the Kankakee River. It is a public-use recreational facility with Section 4(f) protection. Construction of the new bridges and new 4-sided concrete box structures required the temporary closure of the Levee Trail. Mr. Cleveland performed all coordination with owner of the Grand Kankakee Marsh Park (Lake County Parks). Mr. Cleveland prepared the Section 4(f) De Minimis request and obtained successful review by INDOT Environmental Services. The Section 4(f) De Minimis was then transferred to Parsons Transportation for final execution with Lake County Parks. Corradino has email documentation showing the transfer of the completed Section 4(f) De Minimis to Parsons Transportation, so that INDOT could successfully transfer the responsibility of adhering to the requirements of the De Minimis negotiation to the DBBV contractor (Walsh Construction). <br />I-69 Section 5 from SR 37 to SR 39; Monroe and Morgan Counties, Indiana - Provided environmental planning services. He was the project Environmental Compliance Manager. Mr. Cleveland managed the permit application process, coordinated with resource agencies, coordinated between the contractor and resource agency permit modifications, led field reviews with resource agencies, and coordinated other environmental issues. <br />DAVID CLEVELAND, PE, PTOE <br />VICE PRESIDENT, <br />TRANSPORTATION ENGINEERING
