<br /> <br />Page 6 <br /> <br />conduits in the deck pour and shall incorporate the 4 ea. 1” anchor bolts and anchor plate. <br />The anchor bolts shall be held in place with an anchor bolt template until the concrete is <br />placed and cured. <br />6.9. After Deck initial Curing, form and place the 32” cast in place traffic railing. Forming <br />shall incorporate the installation of the double 4” FPL conduits. All forming and <br />placement will be in accordance with FDOT Standard Specifications for Road and Bridge <br />construction and Index No. 423. Slip Forming is not expected to be used for traffic railing <br />due to the limited quantity on this project and interference at the light pole pilasters. <br />6.10. After placing, curing and stripping the 32” traffic railing, install the Bicycle Railing and <br />the 1½” ADA Grab handrail. All manufacture and installation will be in accordance with <br />FDOT Standard Specifications for Road and Bridge construction and Index No. 423 and <br />822. <br />6.11. Apply Class 5 Coating on all exposed concrete, with the exception of the bridge floor <br />6.12. Provide Class 4 Bridge Floor Grooving, in accordance with Section 400-15.2.5 of the <br />FDOT Standard Specifications for Road and Bridge Construction <br /> <br />7. Roadway, Utilities and Restoration <br />7.1. Removal of existing to bottom of subgrade at each end bent <br />7.2. For Roadway section up to each approach slab, install Tensar BX1200 Geogrid and Install <br />and Compact 24” of embankment; Install Tensar BX1100 Geogrid and Install and <br />Compact 12” Stabilized Subgrade; compaction to 98% max density per AASHTO T-180; <br />Install and compact 4” Limerock Sub Base; compaction to 98% max density per <br />AASHTO T-180; Install and compact 5” ABC-3, compacted in two lifts with tack coat <br />between lifts and Install 2½” Asphaltic Concrete in two lifts. <br />7.3. For the Paver Brick Sidewalk, excavate to bottom of sub grade as necessary, install 12” <br />compacted subgrade grade, 8” compacted Lime Rock Base Course, maximum 1” sand and <br />place brick pavers to match existing. Pavers to be installed with a cast in place header <br />curb that matches the existing. <br />7.4. Match and restore existing sidewalks with a minimum of 12” of compacted subgrade. <br />7.5. Re-rod all disturbed areas with St. Augustine Grass. <br />7.6. Install 3 ea. precast concrete light pole bases outside of bridge <br />7.7. Install all water control structures, storm drains, catch basins, inlets, exfiltration trench, <br />yard drains, pipe, tie-ins, concrete aprons, frame and grates, etc. <br />7.8. Core drill and tie into existing storm manhole <br />7.9. Install Type “F” Curb and Gutters <br />7.10. Install FDOT concrete Barrier Walls past the MSE walls awith FDOT index 430 crash <br />cushions <br />7.11. Replace existing herringbone brick paver crsss walks <br />7.12. Install type “D” curbs <br />7.13. Install 9 ea. the Single Arm Light Poles, in accordance with the design specifications. <br />Includes all necessary grounding, conduit, pull boxes, mounting bases, banner arms, etc.
