<br /> <br />Page 3 <br /> <br /> PROPOSED SERVICES: <br /> <br />1. Furnish and Install 1 ea. 42” demonstration Drilled shafts consisting of full length 42” I.D. leave <br />in place steel casing. The Casing for the drilled shaft shall be removed prior to concrete initial <br />set and the drilled shaft concrete to be cut off and removed, after concrete curing, to 2ft below <br />the mudline, reinforcing shall not be utilized. Location and length of demonstration shaft shall <br />be determined by the Engineer and Owner at the time contract. Demonstration shaft shall be <br />installed with the same equipment as utilized for the production drilled shafts. <br />2. Furnish and Install 20 ea. 42” Drilled shafts consisting of full length 42” I.D. leave in place steel <br />casing. Casing to be cut off and removed, after concrete curing, from cutoff elevation to a <br />minimum of 2ft below the mudline. Drilled shafts to be installed in two end bents and 4 in water <br />bents. Bent No. 1 drilled shaft length to be 43.09 ft., Bent No. 2 drilled shaft length to be 51.61 <br />ft., Bent No. 3 drilled shaft length to be 86.81 ft., Bent No. 4 drilled shaft length to be 79.85 ft., <br />Bent No. 5 drilled shaft length to be 79.01 ft., Bent No. 6 drilled shaft length to be 43.09 ft. <br />Drilled shaft concrete to be a minimum of 4,000 psi class IV concrete; Reinforcing to be Grade 6 <br />uncoated with 6” cover; shaft inspection is not required, Cross hole Sonic Logging shall be <br />performed by a 3rd party testing lab. <br />3. Ebsary shall provide design, furnish, install and remove the temporary access trestle to provide <br />access for foundation installation. In the event that the trestle is deemed impractical, then Ebsary <br />shall utilize deck barges and push boats to work from the water. <br />4. Furnish, Install and remove all required Turbidity Curtain and Silt Fence <br />5. Turbidity testing and endangered species watch in accordance with the construction permits <br />6. Demolition and removal of existing seawall section that interferes with bridge construction. <br />7. Furnish and install Tensar or Reinforced Earth Proprietary Earth MSE walls for wall numbers <br />MSE Walls MSEW-1A, MSEW-1C, MSEW-2A, MSEW-2B, MSEW-2C. Walls to include all <br />necessary footings and leveling pads. <br />8. All required marine and road Maintenance of Traffic to include temporary signage and <br />barricades <br />9. Temporary and permanent construction fencing. <br />10. Provide for Stormwater Pollution Prevention Plan, to include necessary inlet protection, silt <br />fencing <br />11. Protection of existing structures and vibration monitoring <br />12. Ground Improvement by either soil mixing or chemical grouting from station 20+70 to 21+70 <br />and from 24+12 to 26+40 to a minimum depth of 20ft below grade and a minimum of 10ft <br />beyond he edge of the MSE wall or toe of the embankment slope or up to the property line. <br />Final ground improvement to be performed by the specialty contractor. <br />13. Class 5 Applied finish coating to all exposed surfaces that are not walk surfaces. <br />14. Fabric Formed Concree Slope stabilization at each end bent. <br />15. Form, Pour and Strip 4 ea. in water Cast in Place Concrete Drilled Shaft Pile Caps. Pile caps to <br />consist of 5,500 PSI Class IV concrete and Grade 60 uncoated reinforcing. <br />16. Form, Pour and Strip 2 ea. End Bent Cast in Place Concrete Drilled Shaft Pile Caps. Pile caps
