Furnish and Install 13 ea. precast and prestressed Florida I-36 Beams for the interior beams. I <br />Beams to be installed with the required Neoprene Bearing Pads and set on the CIP pedestals and <br />shall be cast with Class V 6,500 psi concrete. Beams shall be temporarily braced until <br />diaphragms are cast. <br />18. Furnish and Install 10 ea. precast and prestressed Rectangular Haunched beams for the outer <br />beams for each span. Haunched Beams to be installed with the required Neoprene Bearing Pads <br />and set on the CIP pedestals and shall be cast with Class V 6,500 psi concrete. Beams shall be <br />temporarily braced until diaphragms are cast. <br />19. Furnish and Place 8” thick cast in place concrete bridge deck. Concrete to be placed with a2% <br />crown and galvanized stay in place forms shall be used. The bridge Girders shall be monitored <br />from the time of placement through concrete deck placement to verify if the deflection occurs as <br />predicted. In the event that deflection is more or less than predicted, then the engineer shall be <br />notified. The concrete decking shall be raised or lowered to accomdate for deviations in <br />anticipated deflection. Concrete deck shall have electrical conduit installed prior to concrete <br />placement. Deck concrete shall Class IV 5,500 PSI concrete with grade 60 uncoated reinforcing. <br />After concrete curing, bridge floor shall receive V-grooving in accordance with a Class 4 Floor <br />finish. <br />20. Form and place Cast in Place Concrete approach slabs at each abutment. Approach slabs to have <br />a step to allow for 1½” asphalt overlay. <br />21. Structural Asphalt concrete from approach slab to match existing roadway <br />22. Form and place all incidental superstructure concrete with Class IV 5,500 PSI concrete and grade <br />60 uncoated Reinforcing. Incidental concrete includes all beam diaphragms, endwalls, <br />sidewalls, backwalls, closure pours, expansion joints, etc. <br />23. Form and place 32” FDOT traffic railing on east side of bridge with Class IV 5,500 PSI and <br />grade 60 uncoated reinforcing. Traffic railing to be cast with double 4” FPL conduit, conduit <br />provided by FPL but installed prior to concrete placement. Attached to traffic railing is 1½” <br />ADA Grab Handrail and Special Height Bicycle railing <br />24. Form and place special traffic railing on west side of bridge with Class IV 5,500 PSI and grade <br />60 uncoated reinforcing. Traffic railing to be cast with double 4” FPL conduit, conduit provided <br />by FPL but installed prior to concrete placement and include Special Height Bicycle railing <br />25. Furnish Install and Backfill S-101 Drainage Control Structure with approximately 391 lf. of <br />12”x18” RCP and approximately 8 lf of 18” RCP and approxiamately356 LF of 5’x4’ <br />exfiltration trench and one Ditch Bottom Inlet. Core drill and connect to existing drainage <br />structure. <br />26. Furnish, Install and backfill S-102 Man Hole <br />27. Furnish, Install and backfill S-103 Manhole Pipe consisting of approximately 65 LF of 18” RCP <br />28. Furnish, Install and Backfill S-104 Catch Basin Pipe consisting of approximately 15 LF of 18” <br />RCP <br />29. Furnish, Install and backfill S-105 Curb Inlet Pipe consisting of approximately 21 LF of 18” <br />RCP
